L5-S1 Terrible Pain

In brief, I had a surgery at L4-L5 that seems to have done little for the original pain.

From a CT scan of my L5-S1 level, which is thought to be a problem by the doctors. I am trying to gauge how "normal" this looks. I have congenital narrowing of the spine, diagnosed when I was in my 20's. The full diameter of my AP thecal sac is roughly 15-16 mm. The protrusion here measures at about 6mm at its widest and seems to span nearly the entire width of the disc.

I have been suffering terrible sciatic pain, can barely walk, and have been in agony, no longer controlled by opioids in quite large doses. My question is: Could this be a likely reason? I know it's vague, but can't frame it much better. No other major anomalies that anyone can locate. Thank you so much for any input!

Was this surgery a fusion or a decompression? Has your pain increased, decreased or stayed the same since surgery? How long ago did you have this surgery, everything look okay at L4-5? Is this current sciatica on the same side as the original? Is the majority of your pain down the leg, or do you have low back pain too?

What are your Doctors saying, what are they recommending?

Hi Chicco.  Looks to me like the vertebrea on right side of the picture is hitting some nerves.  The left side seems normal.  I'm not sure what an AP sac is.  Good luck!!!
