Do you have a medical card for your insurance? There should be a number on the back of it to contact them, you could call and find a doctor that way. this is a good site for finding a doctor:

I'm not sure you will find the assistance you seek here but I can suggest that you contact a large hospital in your area and ask for a list of Physicians that meet your needs. It's usually wise to continue with the Physician who has your complete medical history and would know your case better than anyone. Regards ....
