confused about making a right decision

Hello,

I am a 40 year old woman.Mother of 2 children . I ha undergone  left breast lump excision(upper quadrant) surgery in 2003 and fibro adenosis was the biopsy result then. After 4 years another lump felt  3’ clock  position  with constant pain irrespective of menstrual cycle. My surgeon did ultrasound scan for every 3 month., Since the lymph nodes are enlarged she did  an FNAC in 2008 and the result again fibroadenosis. So i was relaxed and didn’t go to surgeon for 2 years even though the l felt slight pain in the lump now and then irrespective of cycle. In 2010  again I went to the surgeon  due to constant pain in the lump which was increased in size andshe immediately did FNAC followed by  segmental excision


Biopsy result is as follows –Cut section(6x4cm)shows yellowish and whitish appearance. Section shows breast tissue with cystically dialated ducts with epithelial proliferation,apocrine metaplasia and lobular hyperplasia in a fibrous stroma.

Surgeon suggested 3 month follow up.But just right after my surgery I felt a shooting pain in my right breast upper quadrant and after8 months surgeon advised  to me go through another surgery to remove the newly formed lump.Surgeon says it is better to remove the area than waiting it to turn malignant.I went  to an oncologist for a second opinion and he did an ultrasound scan reveal
  
Fattty breast with fibroglandular breast parenchyma upper outer quadrant
Two 1mm each hypoechoic area upper outer quadran right breast( I see it like 2 tiny mercury drops)
3-4 lympgh node axilla with fatty hilum

And the oncologist says there is no point(no use) doing the surgery for safety and suggests another ultrasound after 3 month. I am feeling constant pain in that lump area and feeling pain in my armpit too.I am  very confused about what to follow.Should I follow the surgeon (who knows my breast condition for more than 5 years) or the new oncologist?Please give an opinion about this .  Sorry for being so elaborate but without the correct history it is difficult to give an opinion.Thanks in advance

Hi again,
I really don't know what other advice I could give you concerning this lump,except to have it removed and tested by a pathologist for an accurate diagnosis.
You have two Doctors who have different opinions.... and in cases like that you could seek a third opinion since you seem worried and confused in deciding what to do.
I am still leaning towards the Surgeon recommendation that knows your health history and is advising you to have this lump removed.
I don't think that your previous excisional biopsy would have caused the feeling of tiredness and dizziness you have mentioned that lasted for months after the procedure.These health problems could be completely unrelated.
To proceed or not to proceed with the removal of this lump it's really up to you to  decide and how comfortable you feel about waiting till your next ultrasound.
I sure I hope that together with your doctor you will find the best solution to this problem.

Thanks for your advice. Actually i was not inclined to surgery since going through it will disrupt my daily life as the previous one has done to me(i don't mind about the lump pain ). I was feeling very tired even months after it .There is a dizziness in my head which is still now which prevents me  even from doing simple exercise such as walking in the treadmill(it makes  me offbalance. ). Doctor says that it is vertigo but it stared after the surgery.Anyway I am planning to do the surgery after the next ultrasound if the hypoechoic area(1mm) is getting bigger.
I was wondering whether the tendency of fibroadenosis and fibroadenoma can co exist in the same person? And if the hypoechoic area is not fibroadenoma then it will surely a cancerous one  or just another benign condition? Can fibroadenoma cause armpit pain and neck area pain ?Also I have slight swelling in the arm on the same side of the right breast lump and another solid lump popped up in wrist area too which causes me pain(doctor says it as ganglion). Is it possible that the pathologist missed out something in the prevoious surgery(left breast lump) and this is a symptom of reccurance?? May be I am overanxious and less informative to ask this stupid question but I hope you understand me .Hope you will give some information .Thanks once again for your reply

Hi,
I agree with bb with her advice.
A fibroadenoma when painful, should be surgically removed.When a person is young,usually benign fibroadenoma are left alone...but when a woman reaches 40 yrs of age,the lumps(even if benign) are supposed to be removed, as your Surgeon who has followed you for five years, has suggested.

Detailed information is fine! (We'd much rather have lots of info, than just a vague question with not enough info to even begin to try to an answer...)

Iam sorry you have been having so many breast issues.

Have you felt more comfortable (less pain and less worry) after the other excisions?

If so,  your surgeon's recommendation (which seems to be the way you are leaning?) is probably a good course for you to follow, even if not strictly necessary for "safety."
