CA 19.9 and pancreatic cancer

Hi,
I have a CA19.9 reading of 178.6, which is well above the normal range. I understand that CA 29.9 is not a tool for diagnosis of pancreatic cancer, and I do not have any symptoms of the latter. My doctor recommended a CT scan. May I know if this CT scan is necessary? Does such a high reading mean a higher risk of pancreatic cancer? I am in my late fifties and do not have any family history of cancer. What other conditions will lead to such a high reading?
