4 year old who strangled himself

My boyfriends 4 year old grandson recently strangled himself. His mother sent him to his room for time out like she has so many other times, when she didn't hear from the child after a while she went to check on him and he had wrapped something around his neck. My question is how would a 4 year old know to wrap something around his neck? And do children that age have thoughts of suicide? I appreciate any answers you may have, this is all such a shock. My son started this when he was 4. I had him at crisis intervention twice when he was 4. He told us he wanted to kill himself and he wanted to bleed and die. Beleive me they know at that age what they are talking about because I seen it for myself. My son tried plastic bags over his head, breathing in harmful chemicals, cutting himself! He even tried killing my cats with a butcher knife. All harmful things needed to be hidden in my house and he needed watched 24/7. Things slowed down for about 1 1/2 years, untill 2 weeks ago. He got in trouble and said he was gonna kill himself. The very next day he wrapped the seatbelt around his neck twice and was pulling it really tight. When I took it off he had big welts around his neck and was very sluggish and his color wasn't very good. I took him back to the hospital and now he goes for his 1st counseling session at a new facility in a couple days. The Phsycologist had told me that this is a sign of a child that could have been sexually abused! Hate to think that could happen to my son because he only stays with 2 other people who I trust very much, but it could!

What a nightmare for this Family, I have heard of children doing this with the cord on the Blinds,I dont know why but the incidence is high,I doubt it was a purposeful thing ,I know behavior in kids can be copied but I doubt he would be old enough to see it on TV, I cant imagine he would be watching .That would be the only other thing had he seen this behavior somewhere.They have had enough shocks I expect there will be an inquest anyway.

OMG! that is sooo awful!!  i really dont think the little boy intentionally meant to "hurt" himself. im pretty sure that would of been an accident!!  i have a 4 year old daughter, and there is no way that she would do that on purpose. my heart goes out to that family. i couldnt imagine that ever happening.... what a tragedy.

How very sad!  I am wondering if perhaps he was being mischievious and accidently got strangled?  I cannot imagine a four year old actually committing suicide.  How devistated everyone must be.
