Need some input...4 yr old girl behavior

This last week, my four year old grand daughter was taking a bath.  During the bath she began sreaming, unlike she has ever been heard.  She ran into the living room screaming that se was going to break her ankle.  Then she began saying she was going to "puke her ankle.  For the next few minutes she demonstrated very unusual behavior.
as if she was dillusional.  She yelled and told her father she didnt a drink of water.  When he gave it to her, she took one sip, spit it out and poured the rest on the floor.  This happened twice.   She then begain grabbing her fathers shirt and screaming, "Daddy take care of me, Daddy take care of me..."  She did the same with her mother.  911 was called.  By the time the ambulance arrived she had calmed down but was crying. The ER doctor seemed to think she was reacting to the pain
It was also said that maybe her temperature dropped or rose too suddenly while she was in the bath.  He prescribed antiobiotics.  This little girl is a very well mannered, friendly, extrememly smart, never cries or acts out.   There has been some discord between her mother and father that I am afraid she has heard  Sine then she has been very whiney, refuses to take a bath which she always loved, doesn't stay all nite with "nana" like she always had.  She says she wants her mommie.  Has anyone every experienced anything like this?  If so, any help you may have for me would help as I am so worried about this sweet child.

I think you hit the" nail on the head" if the parents have been fihghting and she has heard it is very disturbing and children often think it is their fault that theri parents are fighting, it would be a good idea to speak to them regarding their behavior when she is around. Have a quiet chat with your grand daughter for more input. She has you on her side and I dont think it is delusional I would say there is a problem and she is acting out,
