We live in the uk He is in a class of older children because that's how they work here ithey have 2 year groups in one class
His smearing has been happening since I can remember just over the years it's got worse as time has gone past  I honestly thought he would grow out of it but he hasn't
He has a teacher that checks toilets are free before he goes (I had the same thought) the teacher says no one is there and then my son goes in teacher goes bk to class

Are you in the US?  There are ways to force the issue of testing.

First, you said he is in a class with older children.  Many times this can be troublesome for younger kids.   Are you in the US?  Why is he in a class with older kids?  Is this a public school?
Second, many times kids are afraid to use the public bathrooms.  If he is in a class with older kids then, there are probably older kids in the bathroom when he goes.  Any idea when this smearing of poo happens.   I have seen teachers who will not let a child leave the classroom to go to a bathroom during class time and this many times will cause poo problems.  Is the teacher at all helpful?   Why is he in a class with older kids?
And yes, you really, really need to work with the psychologist to get a handle on this.   It is possible that he has something like ADHD for example.  This is something that your school district needs to get involved in and do some testing.

Oh my goodness.  Very sorry to hear.  Well, that the school is seeking professional help and evaluation, this is a good thing.  I would call your pediatrician about this as well to let them know.  Many children do urinate at night when sleeping at that age but the vast majority are otherwise, fine to go to the bathroom.  Some children suffer encropsis and severe constipation that makes them have issues with bowel movements to where they don't know it is coming and will go in their pants.  So sad when this is the case.  But you describe something a bit different.  A willful desire to smear it.

I think you need to rule out anything medical that could be going on first.  Then work with the school psychologist and your own (that your pediatrician may refer you to) to help him.

Does he have any other behavioral issues?
