There is also the outside chance that he has Sensory Integration Disorder or SIDs.  You might want to post your question here http://www.medhelp.org/forums/Sensory-Integration-Disorder-SID/show/1396 .
Does he have any problems with clothes, sounds, etc

Sometimes pressure on children to make them eat makes it worse , why not take him to the store with you let him chose some healthy foods and maybe a treat, give him plenty of time in the morning with breakfast so many children hate being rushed and dont get break fast .When you have put the food up, leave him to it, no telling him to eat, let him be in control..good luck

What do you offer him to eat, does he eat by himself, children like to do things for themselves, so place the food down in front of him, then no words let him eat or not, if you get on and eat yours he may copy also If you may a big deal of it he may not as he is getting attention from not eating. Focus on playing with him,at other times plenty of outside exercise, there are some great finger foods for them to eat at this age, put him and yourself the same thing each, no talking.. you eat yours and see if he will follow, if he wont eat dont try to encourage him, leave him to eat at his own pace, you go wash the dishes.
