If it is a recent onset disease then the mucosa of ileum and caecum may appear fine in between flares and after taking 5-asa. However, a long standing inflammatory bowel disease brings about mucosal changes that are evident even after the therapy. Please discuss with your doctor. Take care!

My mother showed similar symptoms. Usually bloody diarrhea mostly refers to ulcerative colitis. She had colonoscopy for about 3 times and there was no signs of ulcer, but there was some swelling near the anus. The doctor told that it was proctitis, and treated her. She continued with the treatment for about 2-3 years, but nothing improved. She again had colonoscopy and histological examination inferred that it was ulcer. She is being treated with Mesalamine and now she is well. Please check with your doctor. Thanks
