I will take antibodics all kinds, 2 or 3 back to back, one ent told me that i need  sinus surgery to open up my passage ways they said they would cut the bone. but another ent told me it wouldnt do anygood because it is not my sinuses but my allergies, that i need to fix that problem,  I take allergy medicine everyday, i was taking claritin now i take some new pill umm xylar,  xylm, i dont know but my dr said it was stronger than claritin and zertec.  i cant take steroids anymore.   they was giving them to me to help my sinuses, but last year i had a allergic reaction to the sterioid, predison.  so i cant take anything with steriod in it.  the same ent that said surgery was not good and that it was allergys that i needed to fix also said i needed to take the allergy shots but i have any so many people tell me that after years of taking them every week it didnt help.  i am allerigic to everything they tested me for- but not food, just the environmetal stuff.
but right now, my face is hurting soooo bad, my teeth all the way down to my lower jaw.  my eyes, my ears hurt, i feel like theyabout to bust.  i take sudifed sometimes but i also take provigil for narcolespy and the pharmacis said not to take too much cause they both stimulants.
my meds are:

I would try another ENT.  I had sinus surgery about 10 years ago.  I was just like you, and I also have allergies.  The sinus cleanse things will not work if you are so backed up, nothing will come out, just water.  The surgery I had was done with a laser and they also fixed my deviated septum.  I didn't have packing in my nose, but was very stuffed up for about two weeks after because of swelling.  All in all, it was really not that bad, I came home the same day.

I don't know if that's what they would do for you, but it sounds like you need surgery to me if the CAT scan shows you are blocked.  Actually, I might have to get mine done again because I'm starting to get sinus infections, not as bad as before, but I do get them.  I need to get a CAT scan again, but I would not hesitate to do it because it really helped for almost 10 years.  The doctor told me that some people are just susceptible to getting blocked sinus.

Sounds like you've tried everything the wonderful docs have to offer, so now you can try something different.  Cursor on the blue "friggy" and go to My Journal.  It will give you instructions for The Sinus Flush.  It's risky, because you have to bend over in a shower, but walking around with little sleep and frustration is more risky.

The flush will wash out all of the bacteria, fungus, and allergens that are thriving in your sinuses.  Usually it takes a week, but in your case, probably two.  You will see the effectiveness of it in about 3 days.

Then ask your doctor to try Singulair, if it doesn't conflict with your other meds.  While you are on the allergy drugs, start taking some nettle leaf, from your local health food store.  It ends allergies in about half of the people who I know that have tried it, and doesn't work much for others.  Still a pretty good chance.  At any rate, there's no known side effects.

Once your sinuses are clear, you can go back to your doctors and narrow down the allergens or causes.  Good luck
