Sinus pressure/issues after maxillofacial surgery

About 2 years ago I went through a procedure that I believe was called maxillofacial surgery to correct my bite. My jaw was broken and reconstructed in six places between the upper pallet and my lower jaw. Since this surgery I have had some issues with my sinuses. Between the frequent pressure and the fact that they don't seem to properly drain, and are constantly getting plugged. It becomes very difficult to blow my nose as the boogers are very thick and hard and almost look like scabs. For a little over a year now, the discharge has been a fairly bright yellow color. I have also been blowing blood out of my nose frequently (mostly in the morning when I wake up). Also, I am waking up everymoringing with a sore throat and hacking up yellow/brownish flem (phlegm) out of the back of my throat.

To describe the pressure, it is frequently felt under my eyes on my cheek bones, and in my front teeth (yes I know that sounds weird). The best way to describe it is a tingling sensation, slightly uncomfortable and can sometimes be painful, and when I breath or sniff it feels as though my front teeth are getting sucked into my head. Or it sometimes feels as though there are air bubbles trapped and moving around in my gums. It can also be very difficult to chew as the right side upper pallet can be very sensitive and painful when I try to chew.

I guess my question is, should I be conserned with the constant pressure, discomfort, yellow discharge, and now bloood? I was supposed to return to my surgeon after one year from the procedure for a check up, but lost my insurance coverage, so I never made the appointment and am now a little concerned.
