bleeding in middle ear canal

I had a severe ear and sinus infection and was taking antiobiotics.  I flew on an airplane and experienced severe ear pain, that continued for several days.  Each time I blew my nose, I could hear whishing noises even though my hearing has been muffled.  My low grade fever began to rise up to 102, and then we flew back home.  Again I experienced severe ear pain during takeoff and landing.  Returned home and saw my GP and he said I still had a severe ear infection, and that the fluid behind my ear drum was filled with blood.  He suspects some small broken blood vessels in middle ear canal due to the airplane.  He referred me to an ENT doctor, but wants me to be on different antiobiotics for a few days first.  My ears hurt, my sinuses are still congested.  I see the ENT doctor on Monday.  What will he do during this visit?  I'm afraid that the exam will be painful.  Could my sinuses be blocked also?
