Audio amplification in left ear

This problem started in late 2006 so it's been quite a wile and may be related to a new problem(?)...  Hearing test a year ago said everything is fine. However that is with sound going "into" the ear.  My problem is sound going Out of the ear where everything is amplified (loud), Breathing, Pulse, Chewing (murder with crunchy things).  The new related(?) problem; Had a heart attack this year resulting in a double bypass and naturally many medications.  Recently a problem where my head would feel like it's going to explode from pressure is being experienced the rest of the body is fine, it's just the head where the pressure is felt. This starts every day between 4 and 6pm and have already eliminated medication as a cause by changing the time I take them. Reason I think the two could be related is the thought of a possible tumor near the ear but this comes from pure guess work from me.  What happened in 2006 is I was working in a loud factory setting and leaving work I pulled an ear plug out without relieving the vacuum behind the plug resulting in a quick and very sharp pain in the ear drum.  About two weeks later is when the amplification of noises going out of the ear started.  Three months later is when I seen a general practitioner and he couldn't see anything in the ear.

  So, the question is, what are a few of the possibilities for this amplification of sound going "out"of the ear. And, the head pressure may or may not be related but thought it might help to mention it.

  Not exactly sure what one sounds like in a barrel but hearing ones own voice in his ear is the big problem I have when trying to communicate with others.  After 3 years I hope nothing has been damaged because of this.  Thanks for the heads up and I'll let my general practitioner know what you pointed out and we'll go from there.
