If it was due to the operation one would expect the problem to be apparent right after surgery and the eye not acheve good vision. So it likely is a new problem. On that many drops his glaucoma is probably moderate to severe.
Your father may need some specialized tests like macular OCT to rule out cystoid macular edema.
If the surgeon and a retina Eye MD can't figure the problem and it persists ask for a referral to a neuroophthalmologist.

I just did doctor and the ophthalmologist agreed that there is nothing with the eye he mentioned that there might be a problem with the blood supply to the eye and he prescripe asprine to him.
my dad also takes xalatan, cosopt and alfagan the full dosage. as he has glocoma
honeslty the opthalmologist stated cleary that he dont know what is the reason of this vision loss and he is one of the top ophthalmologist.
what do u think had gone wrong is it the operation or a side affect for the operation
Reasons are too many to list. Since the "eye doctor" couldn't find the cause your father should see a second opinion from an ophthalmologist, ideally a retina/macular specialist. Don't just accept the present situation.
