Temporary Scleral buckles

Why isn't this offered more? The permanent buckle imposes so much suffering for the patient. It's silly to continue this practise when studies show this has similar success rates to conventional buckling!

Hi,

This Friday I am due to have a my scleral buckle removed at Moorfields eye hospital in London UK.

I had the buckle fitted aged 10 & am now 31.
There is no serious pain or infection & I have perfect vision.

Occasionally the buckle causes slight discomfort & so I decided to ask an expert about the possibility of having it removed.

The nurse at the hospital did say the buckle appears to have loosened & the specialist doctor was very relaxed about the removal procedure.

I'd like to know more about the procedure & what will be removed exactly?
What are the risks & what is the recovery time for such an operation?

I understand that every case is different & I'm in good hands at Moorfields but still, I can't find any up to date information on the removal of scleral buckles and most sites I have seen say that removal isn't a common thing.

Has the technology/procedure for removal improved in recent years?

I eagerly await your response.

I plan to do the surgery but the surgeon wants me to understand the benefit and risk first.
Do you think that if this is still worthwhile to try the result of the adding contact lens ?

As you mentioned many RD patients get the eye pain problem, do they normal have a positive result with a cataract surgery ?

The pinhole correct refractive errors and seeing 20/25 is a good sign.  You probably have some astigmatism in addition to your myopia.  Your blur and phoria may also be contributing to your headaches (asthenopia).  I would suggest you get your cataract removed and fix one problem at a time.  The clearer vision may improve you phoria over and headaches over time.

Thanks a lot for you explanation.
Just want to clarify. When you say 'Many patients', do you mean many of them have the headaches or many patients like the one I mentioned, the headache is significantly reduced by the  cataract surgery ?

That cataract surgeon I met yesterday also told me to test with the contact lens first before making the decision of the cataract surgery. But he didn't explain much. But I think my blur vision cannot be corrected much by glasses or lens. I am using a -3.25 glasses for my left eye which I can see 20/60. I was examined by a few optometrist, eye doctors in HK. The best I can see (almost guess) is 20/50 with -4.75. All of them think that I don't need to change the prescription as not much difference.  But I can see 20/25 with a pinhole quite clear. They think the vision problem is
Yesterday, I bought and put a temporary contact lens with -1.5 behind the glasses, my distance vision is still the same blur. I tried 2 hours and find my eye is a little dry and just took it out.

Dr Oyakawa :  Even with the contact lens, anything beyond 30 ft is very blur. FYI, my right eye is -1.75 and 20/15. Do you think is it worthwhile to try the contact lens to see if it will reduce my headache ?

The eye doctor also find me have a mild phoria but only without the glasses. They said that my eye will behave more normal after putting the glass on.

Many patients.  You will have problems if the eyes are more than 2 to 3 diopters different.  I had a cataract that progressed to 7 diopters of difference.  This can be tested by using a contact lens.  If it relieves the symptoms then the anisometropia is the cause of the headaches.  Also, there might be a mild phoria which can be fused but can cause headaches.  I had a mild vertical phoria which was treated with prisms then after almost a year treated with surgery (you really need a good strabismus doctor).  Mine was only 3 to 4 diopters of vertical prism.

Dr Oyakawa once pointed out that a buckle too tight may have other symptoms. I went to my surgeon 3 months ago, he totally disagreed that my symptoms is anything related to the buckle and asked me don't imagine a problem myself.
I got some 2nd and 3rd opinion from other eye doctors. Their opinion is that the pain can be caused from the buckle or from the vision mismatch of the 2 eyes, especially my surgery eye has cataract (most likely caused by the air bubble). They suggested me to fix the cataract to even the vision first before thinking about the buckle.
I also know a guy from a RD newsgroup. He also had some eye strain and used to think this is result of the buckle sequeezing his eye. But he found the feeling disappear after fixing the cataract.

Dr Oyakawa: Most of the people after the buckle surgery with vitrectomy with have cataract plus a jump in myopia. From your experience, how many percentage of these people with have eye strain, pain,  headache ? How many of them will be like the guy I mentioned,  the eye pain can be significantly reduced by fixing the cataract ? Is he a normal case or exceptional case.

I met a cataract surgeon yesterday he raised this question to me. He wants me to understand the chance of getting the benefit I want before I go ahead for a surgery.
