What is best IOL option for buckle eye-nearer or intermediate?

Dr. Oyakawa
Now I am confused!  I am soon to have this IOL cataract surgery on my right eye which is the vitrectomy Erm/ buckle eye in the next few days.
My surgeon has recommended setting a target for my right eye, to about -175 which is similar to the left eye presently. I would not then have a great discrepancy between the two eyes assuming the surgery goes well.  I am wondering if this is a near vision or intermediate vision target?  As I am having a monofocal lens, would I see well enough for nearer tasks and then on the other hand would I only see up to about arm's length with the right eye before things get blurry, so would I then be able to walk around the house and see reasonably without glasses for detail and see the ground/stairs when looking down with the right eye?

Would it be a better option for me to have correction to a more intermediate range and what target would that be? About -150 or -125? I am used to wearing glasses for distance correction for the left eye but I can see well even without glasses with the left eye from a range of about 12 inches to about 10 feet away and just not see finer detail such as TV or read signs without glasses. Eventually my left eye may need cataract correction too although not in the near future. With the surgeon's present recommendation I hope that a nearer target will not leave me feeling too restricted in the intermediate range with my right eye and yet I can still do tasks in arms reach.

Your comments would be very much appreciated. Any other comments appreciated.

The target for IOL after a buckle is more difficult than a standard cataract.  Holladay II has a formula to account for a buckle and it is pretty good.  What is your visual potential in the right eye?  Have you had a PAM or RAM test?

You might consider setting this eye for distance (plano or zero) if the visual potential is good and this was your dominant eye.  This would allow mono-vision.

Thanks for the reply. I did have a PAM test and potential acuity in right eye is 20/30-1. The axial length of my eye is 25.53 if this is useful info.  My current acuity is 20/400
My refraction readings are -2.75+0.25x15 Add +1.50. My left eye is -1.75+0.50x30 Add +1.50

As I used to wear glasses for distance vision only (especially the left eye) and am used to near vision without glasses prior to the retinal detachment that could be why the nearer setting was suggested for reading purposes. My right eye has nearer vision than the left. I need to have some vision correction for the left eye for driving and distance acuity.

I am not sure how to tell if the right eye is the dominant eye.
Also by monovision do you mean seeing the same distance with both eyes?
If I have the plano setting up to how near would I see before needing to have vision correction ?
Would any possible distortion in my right vision be better at near or far distance?
Thank you again.

Just to add I did some research on monovision so know what it is. I am not sure I would be happy or comfortable with the monovision if I opt for plano in right eye as I have had headaches with the present situation with the cataract in the right eye and seeing everything blurry near or far in comparison with the other eye. Also some lack of depth perception.
Perhaps mini monovision?  Do you have a suggestion?

I had mentioned 1.50 or 1.25 previously but can you tell me how far I would see with either of these diopters?

It seems my left eye is the dominant one although still not quite sure but thats how it seems..

I replied just previous to this with PAM result right eye = 20/30-1 and my refraction readings and right axial length.
Anyway would really appreciate yr comments as I am having cataract surgery tomorrow on right buckle eye.

I am thinking that down the line I may have to end up with IOL in left eye if need vitrectomy as already have floaters increasing in that eye which has previously also had a retinal detachment, but not buckle like the right eye. Therefore I would likely choose a distance lens for that eye especially as it seems the dominant eye.

I would also appreciate yr commenting on the possible distortion of vision in my right buckle eye whether worse off with near or distant vision.

This is nerveracking going into surgery with these questions as you can imagine and so hoping for a little more clarity thanks to your informative input.  Thank you.
