PCOS....can you explain it to me?

My sister in law has never been able to get pg.
She ttc during her first marriage of 6 years with no luck.
During her dating period, she seldomely used protection, and never got pg.  She is now married again, 33 years old, and desperately wants to get pg.  

She is overweight...weighing more than 200 lbs.
She has very strange cycles...like 60 days long, 90 days...
She never has a 28-35 day cycle.  When she does have a period, it is very heavy.  I have not noticed or discussed w/her whether or not she has excessive hair growth.  To my knowledge, she does not. She also does not have acne.  

She is seeing her ob/gyn next Tues for a yearly pap, and I don't know if I should encourage her to ask questions concerning PCOS.

So, could she have PCOS, and not experience acne or excessive hair growth?  Would clomid 'solve' her fertiltiy issue if this is PCOS?

This is how I understand it...

She has definite symptoms of PCOS even without the hair growth and acne.  Not every patient will have the same symtpoms or all of the symptoms.  I believe a pt with PCOS can start taking Metformin to help with insulin resistance...and then perhaps move on the Clomid if that doesn't help alone (but it depends on the Dr/RE and how aggressive s/he wants to be).  An ultrasound of the ovaries can help make a diagnosis.

Also, ask your SIL to ask the doctor to have her thyroid levels checked (including thyroid antibodies).  I know I preach about thyroiditis all the time, but it goes hand-in-hand for a lot of people who have PCOS and vice-versa.  Also, having hypothyroidism can cause people to be overweight, have irregular periods, and infertility.

By the way, when your SIL's lab results come back in...have her request a copy for herself!  Some doctors will not treat subclinical thyroid disorders and if her thyroid level is elevated even a little bit, that can be the cause of her infertility.
