Mirena - Would like doctor's advice/help

Hello, I'm a 33 year old mother of one.  I've had a Mirena IUD since November of 2004.  I have not had a period since having it put in.  Yesterday, I started "spotting", but it has now stopped.  Is this something that I should be concerned about?  The only other time I experienced this type of "spotting" was when I was pregnant.  Any advice/help would be greatly appreciated.

Hello,

It is quite common to have amenorrhea (the absence of menses) while using the Mirena.  You should check to make sure you can feel the strings.  If so, and in the absence of any other symptoms, you should feel confident the IUD is in proper place, providing protection against pregnancy.  If you cannot you should see your ob/gyn to have its placement confirmed.

Spotting can sometimes be the result of an atrophic endometrial lining (the inside lining of the uterus), which can result from prolonged exposure of the endometrium to progesterone.  This is the normal function of the Mirena IUD and is not a cause for alarm.

Best regards,

Dr. Downing
