Preg. and generic birth control

Background. Have endometrosis (sp) really bad.  I have a daughter who just turned 1 yesterday. I have been having problems since I gave birth to her.  I was on 2 pills a day for a week than 1 pill to try to stop my bleeding.  Week of May 7th I took a home preg. test and it was positive.  I went to the dr the week of May 21st and found out by ultrasound I was about 6 weeks preg.  I was asked by the nurse, not dr, if I was taking generic brand of my birth control.  She said I was the 4th person to come in that got preg. off generic brand.  Is that possible?  And shouldn't someone be letting woman know that this is happening?  I am married and 29 so it's not that big of a deal that I got preg agian.  But for someone that is 18 on the pill for birth control only, they might not be ok with that.  Plus should I be worried about getting preg. while taking two pills and how accurate is the ultrasound for due date since I am not sure when I got preg?

I am 7 weeks pregnant and no signs of baby. Only a Sac but not baby.
I am having dark spotting, I should schedule a D&C or I should wait a bit.

Where you said that pregnancies coneived by pt on the pill was a result of missing pills or delyas in starting active pills....For me that isn't the case.  Because I was bleeding all the time the dr put me on two pills a day for a week at a time than back to one pill for a week.   The weeks that I took two pills, I took the first one in morning and the second one at my normal time of 5:00 p.m.  ( I set my cell phone alarm for 5:00 so I don't forget-hard to remember sometimes with a little one).  The week I took only the one pill, it was taken at 5:00.  Plus I didn't take the placebo pills, I would start a new pack.  My dr was trying to get the bleeding to stop and me not have a period at all.   It wasn't working.  Before I got preg. the eight months since I had my daughter I basiclly was bleeding every day.  I probably had gotten a month off total in the 8 months.  Around 6 months of that is when my dr told me to take two pills for a week.   After doing that for about 2 1/2 month-3 is when I got preg.  
I don't understand how I got preg. while taking two pills 2 weeks out of each month for almost 3 months.
