hiv tests and oral sex

dear doctor,

i had an exposure to hiv with a SW  in bangalore 39 days back. the exposure was protected vaginal sex and unprotected cunnilingus and fallatio for about 15 sec each max. the mistake i did was to brush and clean my tongue before cunnilingus and i am uncircumcised. after about 10 days i started getting rashes in my genital region. so took a proviral pcr dna test on 18 day which came back not detected.  from 15th day to 31 day i had no symptoms, now my neck lymp nodes are swollen ( confirmed by doctor) and very mild sore throat. on 36th day, consumed by extreme anxiety i confronted the SW and convinced her to take a hiv1/2 ag/ab combo cmia test which said non reactive with a value of 0.36. the logic behind this was if she is not infected then how the hell can i get infected as i am not at all sexually active.

the questions are-
1) can this 37 day combo test of the SW confirm that she does not have hiv.(assuming she was infected before she was with me). dont you think if she was infected before she was with me, the test after 36 days should have become positive? the -ve could also be due to recent infection which should not concern me. what should concern me is a false -ve if she was indeed infected before 36 days.
2) is my proviral DNA PCR qualitative test done at 18th day of exposure reliable?? i am hearing different window periods for dna pcr from different people. everything from 3days to 28 days. please clarify this.

anxiously waiting for your help.

Hello again,

1)what i wanted to ask in my first question was is there a possibility that the SW was hiv+ before 36 days of the test and the hiv duo test to show a false negative.

If she tested 36 days after her last sexual encounter with an HIV DUO then she is most certainly HIV negative

2)and also doesn't my brushing teeth and tongue before cuniilingus increase my risk if she is indeed hiv +?

Possibly but practically no

3) u said pcr qualitative test 18 days post exposure can be taken as certain. is that what u meant in ur reply??

Yes, of course.

best regards, Sean

Good afternoon,

I'm not entirely sure I follow question one but I think you're asking if she was negative is that a reliable test. My answer would be that the HIV DUO is likely to identify a new HIV positive from around 12 or so days post exposure so I think the chances of this woman being HIV positive when she had sex with you are near zero. In any event cunnilingus is a miniscule risk activity and receiving fellatio is a zero risk activity so you are unlikely to be HIV positive from this event.

As far as the PCR test is concerned you can rely on that as certain.

I don't believe yo to be HIV positive. You can check this again by having your own HIV DUO or Combi test from 28 days.

best regards, Sean
