1 hour old tender,pink,non-bleeding wound = open/closed wound? HIV risk?

Hello Doctor, thank you very much for your services here. For past three days I am in miserable agony. I have been advised by members but hopefully an expert advise will put my mind to rest.

I made the mistake of visiting a commercial sex worker (No Sex but following exposures ). Please ***** the risk and advise me about following exposure:-

1. Around 1 hour after I was biting the skin around fingernail (skin not bleeding but inner pink tender skin visible) I made the mistake of fingering a commercial sex worker (from Thailand).
2. Also as I get calluses on hands after pull ups. I press and cut these skin calluses with nail clipper, leaving tender pink skin exposed (painful, sensitive if touched but not bleeding). I did this the same afternoon and later I touched her body and outside of vagina.
3. Squeezing of her nipple with lips with a recently(one day before) healed sore under the lip (little white but not paining).
4. Wet frotting/rubbing her (wet)vagina over my wet underwear. No Sex. But my penis had some skin inside due to masturbation.

-Kindly ***** what is the risk involved with this exposure.
1. Was PEP mandated? Doctors advised differently with one Doctor suggesting PEP is your own decision as this exposure falls into low risk. As it went over 72hours post exposure and as I am thinking over it I am getting more and more miserable.
2. Should I refrain from all sexual activity? The bigger concern for me are the loved ones I am responsible for. I know I made a huge mistake of acting so much irresponsibly. All this misery for just few waste of moments. Please help me doctor.


What is considered as open vs closed wound in terms of HIV transmission medically? An hour old wound with top layer of skin peeled off, which is tender and pink but not bleeding, is it considered an open wound and susceptible to HIV transmission?

Welcome to the forum.

You had brief but accurate replies (in two separate threads) on the community forum.  Without penile penetration, there is no risk for sexual transmission of HIV.  There are no known exceptions.  In addition, it is statistically very unlikely your commerical partner had HIV.  In most of eastern and southeast Asia, well under 1% of CSWs are infected.  And fingering indeed is zero risk, regardless of open wounds -- unless maybe there were a fresh, deep, actively bleeding wound.

There are no hard and fast guidelines about PEP.  The vast majority of experts would advise you that PEP is not a good idea in situations like yours; indeed, if you came to my clinic and request it, we would refuse to prescribe it. But other physicians may have different opinions, and would leave the final decision up to the patient.  My advice is that you don't need it and should not insist on it.  In fact, I don't even recommend HIV testing, unless you need a negative test result for reassurance purposes.

I hope this helps get you beyond your worries.  There really is no measurable risk of HIV in this situation and you should do your best to stop worrying about it.

"And about chapped lips in contact to any secretion coming from nipple?"  Nobody ever caught HIV from kissing someone's nipples, chapped lips or not!

"I wouldn't worry for this life so fiercely unless it was connected to loved ones."  Of course not.  That's always the main fear.  But it doesn't make the risk any different.

"Should I wait for 6 months for test and avoid contact with my wife?"  Absolutely not.  You should continue unprotected sex with your wife this very day. You cannot infect her with something you could not have caught.  And in any case, the time to definitive HIV testing is 4-8 weeks, not 6 months.  See http://www.medhelp.org/posts/show/1704700

Thank you very much doctor for your assessment. I hope I will gain back my appetite and sleep soon. Just one more question, I am really very paranoid right now I hope you will understand. So such wounds are really not prone to HIV transmission right?
