HIV from condom failure

Dear doc,

yesterday I call an escort agency and they send the escort to my apartment. She was around 25 from eastern Europe. Really clean and neat.

I was wearing a condom, the thickest ones and I started to penetrating her, completely, so I put all my penis inside her, and at that moment, when the whole penis was inside her, I heard a noise and I immediately understood that was the condom that broke. I didn't have an intercourse, because I had just inserted my penis, and after inserted the penis completely I heard the noise.
So I stopped immediately, and I removed my penis from her, and I started to check the condom and it was ok, my glans was covered. But I heard the noise before, so I checked it better and I found out that it was broken, it had a hole on the right side, 1cm/0.40inches of diameter (but my glans/head was not uncovered, it was still completely covered, but there was this small hole that probably exposed the corona of my glans in contact with her fluids).
The hole was distant 3cm/1,18 inches from the head of the condom(the sperm cap).

At that point I lost the erection, so I couldn't see properly what part of my penis was exposed through the hole, if the corona or only my foreskin. Anyway I thrown away the condom that still was not destroyed, but with this hole. From when I heard the noise until I took off my penis, it passed 6-7 seconds, so I had an exposure of 6/7 sec. max from this hole, and I didn't have the intercourse, that's why the condom didn't break. Of course it would destroyed if I would have continue with the sex

I started to shake, she asked "why?" and I said because of the condom failure. She said "don't worry I test regularly" and I said "ok but you're not worried at all" she said "If I have to be worried I will figured out next time when I check up myself"
How many chances do I've to catch the hiv from this episode?
I would say 6 sec of contacts of my glands (maybe, I hope that was the foreskin) with her vagina

You've twisted my message, which was intended to illustrate how low your risk was -- and you're reacting with increased worry, instead of reduced, because I didn't say absolute zero!

Lipodystrophy wrinkles??  As judged by an anxious, medically untrained person??  I don't think so!

When a sex worker is alarmed by a condom failure, usually it means she knows she isn't infected and intends to stay that way.  After all, sex workers are at much higher risk of HIV from their clients than the other way around.

Nobody can say the risk from such an exposure is zero.  But if you're the sort who worries about risks in the one-in-a-million range, how do you make it through life?  If you live in the US, the National Safety Council estimates there's roughly 1 chance in 2,000 per year of death by accident (traffic, falls, drowning, etc, etc) -- meaning your chance of being dead in a year is 500 times higher than that of acquiring HIV from this event.

So do your best to mellow out and stop worrying about this.  For sure stop searching the web for things like HIV lipodystrophy!

Oh, and don't forget your seatbelt…..

anxiety anxiety anxiety..been there.like the doctor said 1 in a million chance.very unlikely you got hiv. just keep control of yourself and to ease your mind get a test in a few weeks.all is good.

Thanks for your answers. I was hoping that the fact of not having an intercourse, but only having my penis inside her for some seconds, would exclude me from having catch the hiv. So there is a really small possibility.
Is it because of the exposure of some seconds?

My worries is that when the condom broke she was not worried at all, she was super calm and she didn't know me at all. I remember her face, and she had some wrinkles close to the mouth, they were expression lines, like I also have, but now I read something about lipodystrophy, so maybe they were lipodystrophy wrinkles.

I'm happy to confirm the reassuring replies you had on the community forum.

It is unlikely your partner had HIV; most escorts (expensive female sex workers by appointment) are not infected, and it is rare for people to lie about HIV status when asked directly.  You were wise to use a condom, but the possibility it failed really shouldn't worry you so much.  Almost nobody ever gets HIV from a single heterosexual exposure; the large majority of heterosexual transmissions occur in longstanding relationships in which one partner unknowingly has HIV.  All things considered, I would put the chance you caught HIV from this event as no higher than 1 in a million.

Since you're worried, have an HIV test in a few weeks.  Stay relaxed in the meantime.  It would be extremely unlikely you caught HIV even if no condom had been used.
