Dr. Jose - reassuring please....

Dr. Jose, good morning. It’s me again, Pedro.

I’m really sorry to coming back to you again with the same question you’ve already replied to me. However, I’m still a bit nervous (actually, I scared!) about the encounter I had 18 days ago.
When you say to me I do not need any testing or need to be worried about what happened to me it means that I did not have any risk at all of getting infected? As I said, I am a married man and not testing can implies a lot of things….
I do understand that fingering without a condom and having been penetrated by a vibrator covered by a condom are both very low risk situations. The only meaningful risk situation I would have would be if the girl had large amounts of blood streaming out from an open wound.
But in that situation, blood streaming out from an open wound would be something visible to me? I meant, it would be possible to me to see such amount of blood pouring out from her finger? It would be something like someone cutting his finger with a piece of glass or a knife where blood will be pouring out?
Also, for the penetration process she used oil to lubrificate my anus. It would be possible having any infected fluid mixed with the oil capable to infect me?
I can remember she handling the condom, the vibrator, even touching myself but I cannot remember any sign of blood at all. After she fixed the belt over her waste and vibrator on it too I did not notice any bleeding process nor blood from her fingers. When the penetration over, I looked at the vibrator and there were no blood there either. I just notice my bleeding around 25 mins later at home.
It has been 18 days since then and everyday I take my temperature every hour to see if I have any fever. Besides that, I am always looking to my body looking for any rash, redness, etc…
I am trying to rationalize that but it’s been too difficult. Could you please reassure me with your opinion about what I went through?

Vance thank you. However I could not find the hepB forum and this thing is worrying me a lot. Please consider my question here. I'll be very very thankfull if I get your/doctors thoughts about it and I promisse never ask this again. Pedro

This is an HIV expert forum. Not that the Dr's don't have knowledge about Hep because they do but you should post in the Hep forum.

Sorry to coming back but I just have one thing left: I never thought about HEP-B as I was too much focus on HIV. However, after reading the internet I realized that HEP-B can be transmitted through saliva. I also never took the vaccines (A/B) I got very worried. Based on everything I’ve mentioned above what would be my chances of getting infected with Hep-B?? Pedro

Maybe I am spending to much time thinking about the "remote possibilities of infection" which is a good sign of anxiety level....I am married man and I am so worried about resume our normal sexual life here with my wife. I fell terrible when I think about the chance of infecting her with something..

See what you don't understand is if we say it was no risk how does any test have any possibility not to be conclusive?

Yes I am still a bit anxious but I just would like to know if the results of my tests could be consider as final/conclusive in the encounter I had. I know you and the doctors here have said the encounter was a "no risk one" but considering the most remote possibility of any infection these test would have shown anything? If the answer is they can be considered as final I am pretty sure I will much more relaxed from now on..
That's the only answer I am looking for..

Pedro read what has been written to you. Dr.Sean never told you to test, he said if you remain worried then the only answer is to test. He said the test will be negative, which it was. He said you do not need a test. You have no reason to criticize Dr.Sean or Dr.Jose. Seek a therapist to help you deal with anxiety.

You really really do not need to worry about this. There is absolutely no risk at all of this resulting in HIV infection.

If you remain as worried as you seem to be now - then the answer is to do an HIV DUO test - which combines HIV 1 and 2 and p24 - at 28 days - this will be negative.

thank you very much for everything...what you are doing is not a decent way to help to put an end on everything...you've told me to get tested at 28 days and when I get back with the results you simply disapear....this is really a nice thing to do with someone who is anxious....

Dr. Sean/José, I sent a new td last Friday asking for your medical opinion about the results of the tests I did take. It was a new td on the doctor's area so I had to pay to post it. I only asked for your opinion about whether the results were conclusive/final or not. For my surprise the td has not been answered yet and it was also deleted from the forum. I have to say I'm a little surprise with such thing. I'm still wondering the reasons for that.

You have sugested for my peace of mind to take a DUO at 28 days. I did take the following:

13th days - 4th generation test = result negative
35th days - 4th generation test + PCR RNA Quantitative (min 20 copies/ml) = res negative

So please I'll appreciate getting your comments back about my results. As I've said, I've paid for my question so I'll appreciate getting the answer for it. Kind regards, Pedro

Dr. Sean and Dr. Jose, this post intends to be the final one in order to close this chapter of my life. I had to do the HVI DUO test again due to some mistake of my doctor here in SP. Because of that I've changed the doctor here and the new one asked me for another test round which I did (I got my blood collected again yesterday after 35 days of my possible exposure). The result of this test (DUO) just came out now with a negative result (I tested negative for HIV I/II + p24 after 35 days of my possible exposure - 5th week). Is this result reliable and conclusive even considering it was done at 35 days and not 28 days as per your suggestion? The second thing is my new doctor also requested a PCR RNA test which also was done. The results for ths test are due to come out tomorrow. Can I expect a different result here than the one I got from the DUO??? Plase I am really for all my anxiety and I am very thankfull for your support and knowledge. Pedro

Tks..that's what I read too...no difference at all between 27 and 28th days. I also found a very good article of Dr Jose explaning the differences between all the hiv tests and why the duo is a good option at 27/28 post exposure. My risk was also described as a no risk situation by Dr. Jose, Dr. Sean and a doctor friend of mine in SP. This is giving me some relief while the result does not arrive. I have to agree to you all about the shame and guilty. I am pretty sure that part of the explanation why we are locked up this insane circuit is our guilty, shame and regrets which can only be treated correctly by a therapist. It is very likely I will look for when all of this has gone. My results come out on monday/tuesday so please wish me luck!!

Dr. Jose says that statistically there's not difference between 27 and 28 days DUO results reliability and conclusiveness. I did take it on day 27 after what the Dr. described as no risk situation and it came back negative. However, as "bottlewater" describes some of us feel guilty of having made some erroneous decisions and anxiety keeps playing mind games. I still cannot fully trust the results, therefore, continue to live with anxiety though it's not as bad as it was before the test. I suppose I'll have to wait till the 3 month mark to finally let it go completely. If there's one thing I learned from this whole experience is that it's not worth going through all this for some meaningless sexual encounter as well as the power of the brain, for good or worse.

Is there any significant difference on doing the HIV DUO (HIV I + HIV II + p24) at 27th days post exposure day instead of 28th post exposure day? Of course difference related to the accuracy and reliability of the results....Thanks, Pedro

Well, all I can say is that anxiety really gets in the way of daily living. I would suggest in find a therapist to talk to or a support group that can help. Everyone is human and we all make mistakes some big some small. It going to be up to you to decide what you want to do, test out to 3 months. Really there is something else that is bothering you maybe it was the type of sexual encounter, is it feeling guilty or shame? Me its guilt and shame, disappointment that I let down the one's I truly love so I punish my self and worry thinking about the what ifs this is no way to go through life is it. Follow the doctor advise and find some support.

i'm in anxiety too, and I've a situation that is NO RISK for hiv for the largest part of doctors...i receive protect bj but I don't know if the condom broke itselfs or not...this becouse i was in a dark room...also i have 2  4 generation duo test (27 and 52 days) and both are negative...can you advice me something to defeat my anxiety?

Hi there. Well I know what you are talking about the "what if-ings?" that is easy to do with anxiety, see you your mind is bullying you into these thoughts. I would say that its time to find a therapist to talk about your worries and help find a ways to help you to figure things out. There is some underlying issues that you need to work through that is driving this fear and I can tell you from experience.

samsie, thank you for your post. I am only worried about everything. I trust on what dr. Sean is telling me but my worries are bigger...in matter of fact, the big problems are the "what if.." questions...it looks endless....I just want to get my duo test after 27 days to see its result...coming negative (as dr. sean has said it will come) I will be much much more relaxed...pedro

Just to let you know that I went today for another HIV Duo test (HIV I/HIV II + p24). This is my 27th day after possible exposure. I'll let you know when I receive them from the lab. Pedro

Dr Sean, I called the lab here in SP and they told me that they are no longer doing the P24 test. The P24 was replaced by the HIV PCR quantitative analyses. Regarding the test HIV I and HIV II they are testing normally. So what they test today is: HIV I + HIV II + PCR...As you suggested me to do HIV I/II + P24 now I am in doubt now about the other test...can I do the PCR instead of P24 with the same assurance and reliability on the final result?

I was just expecting some comments about the HIV test I did. Even being not at the correct time (28 days) I was expecting it could have some meaning....Today it has appeared some red dots on my forehead and I do not know what's that. It would be possible to be any sympthon of rash???
