anxious about encounter

Hi docs,
12 days ago I met a guy online we met and had sex. i was the receptive part. He used a condom. But here is where I get scared...The condom fit loosely, although the base was firm the whole rest of it was loose and i have never seen one fit like that before. We had sex and I grabbed the base of the condom as he withdrew and he quickly went to the bathroom and disposed of it. and washed himself for a long time. I did not see the condom aftwerwards. A week later I asked him if the condom broke and if he was hiv positive and he only answered that the condom was ok. Today I woke up all achy with one side of my body´s lympnodes hurting from the groin to the neck. I am afraid the condom could have broken which is why he cleaned himself so much, plus the fact that he ignored my hiv positive question which I assume was because he has hiv. If something did go wrong, does ars start with lymphnode swelling to later grow into the flu like sickness or does it happen all at once? In my country there is no pcr tests that I know of only 4th generation hiv tests. some with antigens some without. I am scared.
Thanks

The pain in the groin already subsided and and is minimal now. I took a test 4th gen with antigen today. I know its too soon and I will test again at 1 month mark. If the symptoms I had were due to hiv yoiu think the test will show it? Thank you and that was the last follow up question.

Hi there,
I am not sure about the statistics in Colombia for HIV.  I would imagine that the HIV rate among gay men is possibly similar to the UK, which is higher in the bigger cities like London or Manchester.  Therefore if you are in Bogota, this incidence is probably higher than in the rest of the country.
Your groin pain is certainly possibly muscular, but it could also be related to a different STI that could cause groin pain.  You ought to be tested for anal Chlamydia and Gonorrhoea, just in case.  If you get a 4th generation HIV test including antibodies and p24 Ag (the equivalent to our HIV Duo), I would consider a negative result after 28 days definite and conclusive.
Best wishes,
Dr Jose

thank you Dr. I live in Colombia. The pain is now only concentrated in the groin..the other must have been muscular. Ihave an app with a urologist in 2 days.  If I take the HIV test at 4 weeks  post 4th generation with antigen ( I dont know the brand name) will it be definite? Thank you

Hello there,
It is difficult to assess the possible risk in this case as we do not know for sure: 1.) if the condom was intact or properly placed, 2.) this guy's HIV status.  Bearing all that mind and assuming that the condom was not ok and that he was positive, there is obviously a significant risk of HIV infection and you have to get tested.  However all of this seems like too much assuming.  There is no reason why the condom should not have been fine, and statistically the majority of individuals are negative (so this could be the case of this man; I do not know the country you are in, as obviously this information is also important to estimate the risk).
Some of your symptoms can be consistent with acuter HIV sero-conversion, even though they seem to have started too soon.  They normally begin after 2 weeks an not earlier.  If you are having symptoms, you could have a simple HIV test.  Antibodies should be detected if there are symptoms.  A negative result should be highly reassuring.
Best wishes,
Dr Jose
