HIV Risk for Me and My Wife

Dear Doctor,
I had an unprotected exposure with a massage girl in last week of Sept'2011. I rubbed my penis with her vagina with my erected penis from behind. Then I ejaculated in a minute while constantly rubbing her vagina from behind. I did not insert my entire penis in her vagina. She might have dropped her pre-vaginal fluids on my penis, which I am not aware of.

In mid Oct'11, I had symptoms of sore throat, swollen lymph gland on neck, muscle pains. I showed to a Doctor who gave me antibiotic for 3 days and I became normal. Again after 10 days, I had repetition of same ARS symptoms for a week. I did not have fever or rashes yet. I did a complete blood count which showed lymphocytes at 16%. Also I lost 2 Kg weight in a month. I am quite scared and depressed and stressed every day and night.  The pain in lymph node at back of my neck goes on and off these days.

My CD4 percentage came as 27.3% (normal range 32 to 50%). The CD4 count is 852. Also HIV Antibody (Ab/ag) Elisa test at 50 days came negative. I know its window period.  Can I do PCR viral load test? Also why CD4% came so low? Can stress cause changes in CD4 / lymph pain? What are my chances of getting HIV?

Me and my wife have done unprotected sex 3-4 times in Oct'11 but she was using female condom and I did not ejaculate inside her. She had a Vaginal abscess last week, which made me more scared that I might have infected her. The doctor gave her strong Antibiotic for 3 days and she had fatigue and muscle pain these 3 days. She also has pain in her right hand above elbow seems to be Epitrochlear Lymph node pain. What are her chances of getting HIV?

Me and my wife are quite scared and we have a small son.  Please help us.

Your negative DUO test at 50 days is fully conclusive and no further testing is required.I suggest you pay attention to what both doctors have told you as they are experts in these matters.You really need to accept your negative results and move on with your life.All the best.

Thanks Dr. Sean. I was worried that my 52 days was not conclusive due to symptoms of mine and my wife and thinking it to be false negative. But after your reply I am relieved to a great extent. Thanks a ton for this help.

Hello Vishal,

The further questions you are asking are irrelevant. You have had an HIV test and also CD4 and both are negative and normal (I did see your query regarding CD4% and am unconcerned - this is just normal variation - the key here is that your CD4 was very high at over 800). Hi Dr. Jose. I am awaiting your reply on my queries from thread dated 27th Nov.  Also need your expert opinion on whether antibodies and antigens are mutually exclusive at 52 days CMIA Ab / Ag test in an infected person. I mean atleast one of Ab or Ag should be positive at 52 days. Do I need to have an Elisa and PCR viral load at 90 days?

Hi Dr. Jose. Kindly advise on the 3 queries as in above thread. I am really still suffering from lymph node pains and oral thrush. Humbly thanking you in advance with regards.

I have seen many people on medhelp who have ARS symptoms after unprotected exposure and have continued dropping CD4 results and lymph pains and yet not diagnosed by HIV tests or for any other disease. I am worried if this will not be my case also in the future as this affects me, my wife and our kid's future. I wish we don't have a new STI, which is not identified in medical science as was the case for HIV before 1980s.

Hi Doctor, can you please advise on above query. Also to add what would be your assessment if:

1) my penis went inside for few seconds as I was pushing from behind
2) if the massage girl was in menstruation cycle
3) co-relation between my and my wife's symptoms - wife was using female condom.

I am at 60 days post exposure. I am still having lymph node pains in body and continued oral thrush and unable to sleep.

Thanks doctor. Your reply really gave some sigh of relief. You really do something great to lives of many like us.

I am worried because in this act of frottage for a minute, may be my penis went inside the girl's vagina for 5 or 10 secs may be a few centimeter inside, which I don't remember clearly because I was pushing from behind.

Should I do HIV Viral load to put the stress away? The symptoms which I had were physical and not created by my mind. After having symptoms for 4-5 days only had a look at internet. I am worried about lymph node pains which is on & off.

Further I also request your advice on possibility of my wife getting infected from me considering that she got an Abscess last week and also feeling pain in Epitrochlear lymph node. I am worried about her. Could the female condom she was using might have protected her assuming I might be infected. If she will be infected bcoz of me, that will be virtually end of our family life. I am worried about future of my son.

Do you see a co-relation between my and my wife's symptoms? Is it safe to unprotected sex with her now? I am sorry for writing more but this issue is a question of life and death for us.

Hello,
Thank you for your post and welcome to our forum.
I can assure you that you do not have HIV and therefore neither your wife.  
Firstly your sexual encounter did not pose risk of HIV infection because there was no penetration, even assuming that the lady was positive and your penis came into contact with vaginal fluid.  To put yourself at risk, you need penetration, and even so, the risk for the man woudl still be relatively low.
Secondly you had a negative HIV Duo test at 50 days.  That is final and fully conclusive.
Your symptoms are not related with acute sero-conversion.  Your CD4 count is completely normal, and abnormalities in the percentage happen with most infections.  That is what I think you have; a simple infection causing a sore throat and enlarged cervical lymph nodes, compounded with your stress and anxiety.
No need to have any further HIV tests.  You are definitely HIV negative.
