Twice exposure

Hello Doctor I am 30 years M from India. I was having protected sex with a prostitute on july 25 2010. I discovered that the condom broke during the ejaculation and i had an exposure.I  forgot the incident as she pleaded she is not HIV. After 4 mnths i had protected sex with a Facebook friend, she is married 40 yrs F with a 20 yr old kid married to VP of a big corporate house. but during the intercourse she started bleeding and some blood percolated on my testes. I had a boil on the rear end of testes and it was intact at that time...after sex i quickly washed away the blood with warm water and applied antiseptic lotion.

After abt i month from the last exposure i got a recurrent fever which did not crossed 100 and in general it was 99, it lasted for abt 15 days. The Widal test came positive and the Malaria test negative but the fever vanished only when i ate Lumerax 80 (Malaria tablet). My SGPT was raised to 140 initially later after it decreased to 40. The platelet count reduced during fever to 148 now its 230, during infection the abdominal USG said i had mild enlarged spleen.

Afraid of the situation i read a lot on the net and found all the symptoms in me including pains in the testes..I conducted the ELISA HIV DUO ULTRA after 7 months of initial exposure and 14 weeks i.e > 3 months after the last exposure. The test came NEGATIVE...I have following questions

1)Can the HIV infect outside the host body like in my case of last exposure
2)Is the HIV DUO Ultra conclusive? I dont know its 3rd or 4th gen test, can a manual error happen?
3) Do i need to repeat the test?

Hello,
Your negative test is totally reliable and fully conclusive for both sexual encounters.  Therefore there is no doubt that you are HIV negative.  No need to have any further tests.
Best wishes,
Dr José
