HIV+ Encounter, Impact of Injections

Hello Dr. Cummings,
I am a healthy, circumcised male in my 20s. I have never done drugs and this was my first sexual encounter. 38 days ago I only had unprotected oral sex with my former girlfriend along with fingering. 3 days later she admitted she was HIV positive. She’s Ukrainian. Furthermore, she told me she isnt on medicine. Shes been + for at least 10 months if not longer.
The oral lasted 15-20 minutes and was a mix between oral and hand (the hand part was a bit rough) and for some reason, I couldnt ‘get off.’ I ‘finished up’ by using shower soap. I didn’t wash the soap off. The next day, my penis was red, tender, and sensitive. It peeled completely 10 days later. I don’t remember seeing any blood during the act.
Ive had no ARS symptoms. At hospital two injections were given: Rocephin 2mg and Panavir 5ml. They said one would kill all the bacteria in the blood and I think, the other would ‘help’ the immune system, Panavir. Panavir is available in several eastern-bloc countries. Its an “antiviral and immunomodulatory agent, which increases nonspecific resistance to various infections and contributes to the induction of interferon.” It’s ingredients are “purified extract of plant shoots Solanum tuberosum, the main active ingredient - hexose glycoside composed of glucose, rhamnose, arabinose, mannose, xylose, galactose, uronic acid.” Used in treating  herpes, HPV, secondary immunodeficiency state against infectious diseases, & flu.
29 Days - Negative DUO
1) Was soap the cause of my penis being red and tender the following day?
2) Did the soap reduce my chances of HIV or STDs?
3) Could alcohol consumed prior kill the enzymes in her saliva that inhibits HIV?
4) Correlation between strong immune system and ARS?
5) What’s the % of people who experience ARS?
6) How often do you see a negative DUO at 28 days go positive later?
7) Could Panavir’s immunomodulatory capabilities have impacted my results or delayed seroconversion?
8) Negative 29 day DUO conclusive?
Cheers!

Thank you for your reply. Today, I was diagnosed with oral thrush. As of right now, it's just limited to the tongue. I know that this is a relatively rare symptom of ARS. Is it possible a singe injection of Rocephin at 2mg could cause this just over 6 weeks later??? If it was HIV related, wouldn't it also be accompanied by a fever and other symptoms? I'm just a bit concerned as I've never had this before and that something is off balance within me...

Say Panavir did have an affect on my immune system and its response to HIV...if I were to test positive, it would still occur within the overtly conservative 3 month mark, rather than extending it to 6 months, right?

Hello,
I am afraid that it will be me answering your post.  Dr Cummings is not available at present and I thought it would be best to reply to you as soon as possible.  It is not very clear from your description if you received or performed oral sex.  I believe that if the former, and if that is the case, there would have been no HIV risk to you.  Receiving oral sex does not involve any risk of HIV infection, even if the lady is positive with a high viral load.  Fingering is safe too.
I will answer each of your questions here below:
1) Most likely.
2) No.
3) No.
4) It is possible that people with a stronger immune system might tend to have more severe symptoms due to the immune response.  However there are no studies available to back this up.
5) Over 90% of infected people.
6) I have never had this experience if the dates were right.
7) We do not prescribe this medication in the UK.  Therefore I am not familiar with it.  However I would very much doubt that it would have any impact on the reliability of the test results.
8) Yes, it is conclusive.  You do not need any further tests.

Best wishes,
Dr José

After doing a bit more research, I found a bit more out about Panavir and it sounds like it has anti-HIV properties. I guess I'm just worried that the drug has delayed any advancement of HIV thereby undermining my test results.

I came across this website which had a lot of information about it. http://sec.edgar-online.com/powerverde-inc/10ksb-annual-report-small-business-issuers/2005/03/29/section3.aspx

I found statements such as this:

"In laboratory tests the antioxidant and free radical scavenging activity of Panavir ® inhibited activation of HIV-1 in latently infected cells. "

AND

"The Company believes Panavir ® directly inhibits HIV-1 in part by interfering with the attachment of the virus to cells and also by inhibiting the syncytia-inducing strains of HIV-1."

Have you even heard of this drug before? Let alone it being used to treat HIV? In some places I see it referred to as Probucol too, which is confusing me since that's a cholesterol drug.

Do you have any idea how long an antiviral like this would stay in the body? I only had one 5ml injection. According to the company's website, if you want a proper treatment, you go back for multiple jabs within the first week, so I assume it leaves the body quite quickly? Also, on the official company website, I don't see anything specifically regarding HIV, other than it can be used to treat "secondary immunodeficiency states against infectious diseases," which may refer to HIV? Ok, I'll leave it at that.
