Condom Came off during sex with CSW

Hi Docs,
              Thank you for your time and expertise. My anxiety is through the roof. Im a 35 year hetero male.A week ago I had unprotected oral and protected intercourse with a 20 something white female CSW who usually works in Atlanta and NJ. While she was on top of me she had climaxed. Shortly after we moved to switch positions and I noticed that the condom was not on me. She had to pull it out of herself and it was not hanging out. I freaked and asked her if she had anything and she reassured me that she didn't and was rather calm about it which scared me even more. Although I never ejaculated I am extremely worried about contracting HIV. 2days after the episode Istarted getting mild itching in the urethra area that seemed to be alieviated by gently squeezing my penis. I also have mild discomfort or light stinging at the opening of the urethra. I have been urinating a bit more frequently but Im diabetic and my sugar has not been as good as it should be . I have no blisters or discharge after a week but just the symptoms I discribed. What are my risks? Help as I am very afraid for the worst. Thanks

Welcome to the Forum. Your risk of HIV is low.  It is unlikely that your partner has HIV (few CSWs do) and even if she did, the risk for getting HIV from a single exposure is less than 1 infection in 1000. There is no meaningful risk of HIV from receipt of oral sex.  Further, your exposure was at least partially protected reducing your risk still further.

Your symptoms are not due to HIV.  Early HIV symptoms do  not start until 2-3 weeks following exposure, not in just a few days.  On the other han, the symptoms you describe could be due to urethritis (penis infection) and I would suggest that you get evaluated by a knowledgeable professional for gonorrhea, chlamydia and for non-gonococcal urethritis (NGU).  These are far, far more common than HIV and easily treated.  

You may wish to get follow-up HIV testing to prove to yourself that you did not get HIV.  Testing at 6 weeks will detect 95% of HIV infections and at 8 weeks virtually all infections will be detectable.  I am confident that testing will show that you did not get HIV from this exposure.

I hope that this condom mishap will not keep you from using them in the future.  Condom malfunctions of one sort or another occur about 1% of the time they are used but they remain the best prevention for all STDs.  Take care.  EWH

No change in my assessment or recommendations.  Your swollen lymph node, if that is what it is, is not due to HIV.  You should consider getting checked for other STDs.  EWH

Hi Dr.
     my final report on this manner is after reading your post and feeling more assured for most of the day I began having pain in the groin and noticed that I indeed have a swollen lynph node the size of a hard Lima Bean on the rightside. That coupled with my symptoms I expressed to you earlier has me worried about HIV. The lymph node thing really freaks me out can you shed a final though on what I may have as I will be getting tested this week. Thank you once again as this is my final question.

No picture is needed.  This does not sound like herpes. As to whether it is gonorrhea or NGU, you need the tests to tell.  EWH

Thanks Dr for your reply. I will get some tests done this week. As of now no milky white discharge or anything like that just the itching of the urethra and mild swelling of the opening. I can provide pic if needed. It's been 10 days so I guess my last question if possible does this sound like herpes or ghonnoreah or just NGU? Thx so much. I tried to post this reply before but something went wacky with my computer

My account  has been disabled despite being charged $20 for my question.My nick name is iyer100. I have asked you question by nick name mentioned where as i asked Dr Sean by different nick name have i done something wrong??Then why i have been charged money when i cant ask questions, had asked quetion about the STD testing as you have said that HIV is nor my problem,I want to ask do i need testing for STDs.I have been charged money, i m not abusing the forum but asking genuine questions that too after paying for my questions..How can they be disbaled????
