please evaluate risk

Please evaluate my risk - Im am realy scared.

condom breakage with csw in new york city.  she is white in late twenties / early 30s.  sNot sure when broke but assume was at least 5 - 10 minutes.  I freaked out and quizzed her a lot.  She said she uses condoms with everyone (and she insisted on them when we first met) and was not injection drug user - but did some other drugs recreationlly.  Oral was covered.  She said she was cleans and been tested a month previously and was negative.  I had negative test for syphillis, HIV, Gonorhrea and chlamydia 7 days after incident.  I hve a regular partner with who I have unprotected sex.  have been avoiding sex with her since incident, but would like to continue unprotected sex with her.  but definiteily, don't want her to get an std.

based on incident and test results, do i need further testing before safely resuming unprotected sex with her.  please help - so scared.

Welcome to the Forum.  I'm sorry to hear of your condom failure,  Statistically this happens on about 1% of sexual encounters an is a bit more likely to occur with new partners than established ones.

Your risk for getting any STD, including HIV from this encounter is low since most commercial sex workers do not have STDs and most exposures to infected partners do not lead to infection.  In your case, your partner says she gets checked regularly and that is probably the case.  CSWs who insist on condoms are using them to protect themselves, not for their clients' sake.  In addition, you have now proven that you did not get gonorrhea or chlamydia from the exposure since your tests at 7 days would have detected infection if it were present.  Syphilis is so rare among heterosexuals, even among CSWs that I would not worry further. That leaves HIV.  Again, your risk is low.  Your partner told you she was not infected and that is probably the case- few CSWs are. Even if she were, the chance of infection with a single encounter is less than 1 infection per 1000 sex acts, on average.  Thus, if I were you, I would not worry.   Having said that, and from the tone of your post, my guess is that you will want to be sure you were not infected- a standard HIV antibody test taken at 8 weeks will provide a definitive answer, as will the combination of an antibody test with either a p24 antigen test (the so-called 'DUO" tests) or a HIV PCR test when the two tests are taken at 4 weeks after infection.  The abrasions on your penis do not change my assessment.

also please add to your assessment tht I noticed a couple of very small cuts on my penis afterwards probably because of vigorous sex.  also, i have no "obvious" std symptoms.  really trying not to interpret any little issue as a sympom of hiv, but it's difficult.
