MRI results

My 14y/o suffered TBI from an ATV accident. His brain endured too much pressure for too long before anything was done. His latest MRI results revealed extensive encephalomalacia in bilateral frontal lobes. Right frontal lobes extends posteriorly within the right middle frontal gyrus. Encephalomalacia in both temporal lobes, more in left. Right greater than left parietal encephalomalacia. Many of the areas of encephalomalacia have ribbon like areas of T1 shortening. Extensive T2/FLAIR prolongation subjacent to the areas of encephalomalcia. MRI was performed to see if he will ever be able to talk again. Please help me understand all of this.

  What about this & migraines???Can it cause them?? The damages???

Well, without actually seeing the MRI films it is really very difficult to comment. Damage to the frontal lobe of the brain causes frontal encephalomalacia. This lobe is associated with mood and memory. Thus any damage to this lobe can lead to long term memory loss, mood swings, talking too much, and energy bursts leading to over activity. Long term complication is chance of schizophrenia.
Encephalomalacia is not an illness. It is basically a softening of the brain matter as a result of an ischemia or infarction, degenerative changes, infection, craniocerebral trauma, or other injury.  Usually this is not reversible. Treatment is just supportive. If there is a clot blocking the blood supply then this can be treated. If the speech center is involved, then I am sorry, speech may not return. Please discuss with your neurologist.
