When a person coughs, the pressure in the head goes up.  With the injury and surgery, his brainstem is weak and easy to damage.  The coughing pressure or blowing the nose will cause pressure on the brainstem.  The loss of consciousness is something the medical people should be told.  It means the brainstem is still healing and it has been a little damaged again.  Brainstem keeps us breathing - very dangerous if it is injured.

There are nerves in the neck that would cause the face and shoulder problems but the foot problem is not connected directly to this.  If he is walking differently because of how he holds his head or shoulder, he will twist his legs some and cause these other pains.  If it is spinal cord damage, not nerves -- then he is still healing.  Nerve and brain tissue heals very very slowly- perhaps longer than a year.

It is probably not a great idea to be going back to work, driving a car or bicycle, etc.  Should be around people in case of passing out - breathing problems may happen.

He may always have some problems, but the tissue will heal more.  He must not rush this or it can get worse.
