The only way a mitral valve disorder can cause symptoms is when there is a substantial decrease in cardiac output....that would take a moderate to severe reguritation. The regurgitation (back flow) of blood into the left atrium, would impede normal blood flow and blood would back up into the system and fluids would leak into tissues causing the swelling (edema)of the peripherals. If you have the reduced cardiac output from a leaking valve you would have other symptoms as well such as shortness of breath, chest pain, cough, fast heart rate, etc.
Sometimes there is a sharp shoulder pain due to insufficient blood flow to heart cells, but more often there is chest pain. So if MV regurgition is serious enough to cause a reduction of blood flow to the heart cells there could possibly be shoulder pain. Heart failure, kidney failure, or liver failure should be ruled out as well a blood clots within the leg vessels.
