still suffering but stll fighting

I have difficulty breathing when lying down along with severe chest pains ,fluttering ,a recent symptom is shaking uncontrollably right before falling asleep causing me to wake. I have swelling around my abdomen and in my legs I have gained 20lbs in last 4 months I have severe lower back pains and tingling in my head I  feel my brain is becoming damaged by whatever it is I have. I cough up green & black phlegm, and my vision is blurred.I've been to the er 20 times in last 4 months with no answers ,I've been to cardiologist, pulmonologist,GI,still no answers!  Whatever it is has progressed and I feel the end is near. With no health insurance I feel I have been brushed off and bounced around from specialist to specialist until now I'm broke and my body & mind are exhausted. My skin is very pale and bluish,my arms and legs have lost muscle tone yet I still gain weight. I feel this is my heart .I need advice /what do I do???????????/
