Creatine phosphokinase

I know there is three types of CPK.  When your doctor is alerted of elevated cpk should the doctor also be told the type of cpk as well.

cpk bb type 1, cpk mb type 2 and cpk mm type 3

My doctor does not seem to know or is not important?

Well it's just slightly important because it points him to the source of the problem. Perhaps he didn't specify a break down. It's like when my Doc fills in a blood test form, she just ticks the cholesterol box so I shout "Hey, and the lipid breakdown".
Your Doc can call the test lab and ask which type of CPK was elevated.
