Do These Test Results Sound Bad?

After having a 24 hour holter moniter installed, my results were as follows:

> Average heart rate was 92 BPM w/ a maximum of 176 BPM and a minimum of 53 BPM.

> 186 PVC's w/ no bigeminy, couplets, ventricular runs, or PAC's and PAC couplets.

> 4 SVT episodes, totaling 32.6 seconds in duration. Longest SVT episode was 16.8 seconds and highest rate was 143 BPM.

> 172 tachycardia episodes (over 120 BPM), totaling 2.5 hours in duration.

> 22 ST displacement episodes (over 1.5 mm), totaling 27.4 minutes in duration.

> 1 episode of baseline, sinus, ST-T changes.

> 1 episode of rate change.

> 1 episode of tachy/brady.

All help would be appreciated because my cardiologist doesn't tell me much and says it's all from anxiety.

I agree that your heart would be considered structurally normal.  My daughter has had a few echos and some have said there is a little regurgitation, and others times it doesn't even show up.  I highly doubt the valves are a contributing factor to the SVT.  What does your cardio have to say about the study? Regardless of the cause of the episodes, what does your doctor recommend for comfort, etc?  If you can't get answers you are comfortable with, you may want to consider a second opinion from an EP.

Your heart is still considered structurally normal even if you have trace valve regurgitation.  Today's equipment is so sensitive that it picks up stuff that it couldn't years ago.  Your echo sounds pretty good, as episodes of SVT for only a max of 16 seconds and a rate of only 143 would not be worrisome.  You did have a lot of tachycardia which I would assume is sinus, which can be due to anxiety.  I am not a doctor, but maybe you might want to look for another opinion to see if you have IST, inappropriate sinus tach.

When I said I had a max heart rate of 176 BPM, I was referring to a normal sinus rhythm, but when I said I had a max heart rate of 143 BPM, I was referring to the SVT, which is different than a normal sinus rhythm. By the way, the test results on my echo that pointed out my trace regurgitation in the mitral and tricuspid valves said "normal" under "findings." I suppose a trace amount is not considered abnormal, otherwise he probably would have said something.

If you have mitral and tricuspid regurge, then your heart is not structurally normal.  Although, it may not be all that abnormal.  Mitral regurge is very common.  Disturbances of heart rate are also very common in the presence of valve problems.  I would ask your Cardiologist all about the manifestions of anxiety and if he/she can't/won't explain it then find someone who will.  You mentioned that your max heart rate was 176 and that there were 4 episodes of SVT with a max heart rate of 143.  That doesn't make sense.  Your doctor needs to explain the "how and why."

I forgot to mention, my heart is structurally normally. However, I have trace mitral valve regurgitation and trace tricuspid valve regurgitation which my doctor didn't mention to me as being a problem. Is it normal to have trace regurgitation in these valves and do you think they are contributing to my symptoms?

Thanks!
