blood test indicates kidney disease, what should be my next step?

I just had a routine blood test yearly physical and joint pain.  I have a history of heavy NSAID use.

My eGFR is 49
Creatinine 1.15 mg/dL

My CBC shows anemia
Hemoglobin 11.5
MCV 76.2 fl
MCH 22.6 pg
MCHC 29.6 g/dL

My cholesterol is high, absolutely no history of high blood pressure or diabetes.  

My nonspecific symptoms:
Fatigue
Restless legs so bad at night I cry
Hard to concentrate
Hard to think of words
Cold all the time
Dizzy
Hands swell

My dr scheduled me to go over my blood test in 30 days.  Should I try to go in sooner or is it not serious enough to go in sooner?
