I am Suffering from Epididymitis for over a year. What should I do?

I am 25 year old male, I have had Chlamydia once for about 5 months which is now treated. I was given antibiotics to treat the initial bacterial infection. I have seen 10 doctors about this and now I am referred to a Urologist who I will see very soon. However when I urinate it starts to hurt could my Epididymitis be also caused by calcium deposits on or within my epididymitis? I don't have a healthy diet since I eat microwavable and frozen food . Also a high sugar and sodium intake, furthermore I smoke about 3-4 cigarettes a day and I drink half a bottle of wine over a course of 2-3 days per week. The pain on my left side of my testicle is becoming almost unbearable and finding it very difficult to work. I have taken two full courses of antibiotics and endless amounts of NSAIDs especially Naproxen where 2 tablets a day no longer improve the pain, so I have to take 3 tablets so I can sleep at night. Thank-you
