My testicle and back are painful. Very painful. Whay is going on?

My left testicle and lower left bavk hurt so bad. My front left abdomen hurts even more. Went to the doctor for the front left abdomen and lower back pain, and they gave me physical therapy. I did what was asked of me, and I had a minor heart attack a few weeks ago. Physical therapy and massive diet changes. After my heart attack I was so tired and have been dealing with a lot of backlash for not taking time off of work. My job is simple enough, and requires little stress if people aren't questioning my every move.

My left testicle has started hurting so bad, along with the same pain i had with my lowrr back and lower left abdomen issue. My left side hurts, and my lect testicle hurts to the touch but no swelling. There is a striation pattern on my left testicle that was discovered during an ultrasound. My anus is also having an issue with its shutter speed now for some reason. I want to know what is going on, but no doctor will listen. Any questions? Ideas?
