Penile issues

So I have been tested for everything under the moon several times and everything comes back clear. I have seen a urologist who diagnosed me with phosmosis, and was told I can continue to use this anti fungal cream on it, but seemed to make it worse. Now I have a pain in my furenelem if that's how you spell it. But it's white patches of skin and just hurts whenever I retract it. I doubt I have an std but it has to be something because I also have tenderness in my testicals.
