Ridge on front of testicle?

Hey guys -
I'm a 25 y/o white male, no sexual history (so no STDs). I was diagnosed with prostatitis a year ago, following a few weeks of binge drinking. I took an antibiotic (Cipro?) and it cleared up. Back in March, I felt a twinge of pain in my right testicle. I had recently started sleeping on my stomach, and had been working a new office job for about seven months, where I sat 10-11 hours a day. Basically I went from working an outdoors job (grounds in the Alps) to living a sedentary lifestyle (except for rock climbing 3-4 times a week).
I didn't go see my doctor. The pain went away after a day, so I continued on. I felt it every now and then, but it always went away. In August, though, I moved and did a lot of heavy lifting. The next day, I could barely stand because my testicles hurt SO bad. After two days of this, I went to see my GP, and he diagnosed me with epididymitis. He put me on Cipro for two weeks. During this time, I moved to Europe. The first 2 week course of Cipro did not take care of me, so I got another 2 week course. This helped a lot. The pain from my epididymus (I can feel it on the back of both testicles) is gone. They are still a bit inflamed, I think, but they do not hurt and get less inflamed each day.

Now, however, I have pain on the FRONT of my right testicle. Weird, huh? Basically, when I feel my right testicle, I feel a semi-hard ridge on the front of it. It hurts when I touch it, and whenever something bumps up against it as well. I used to masturbate frequently, but I'm trying to stop that as I suspect this does not help the situation (I've been a chronic masturbater for years).

Any idea what this could be? I've done a lot of research (I've had testicle pain for months. You can find out a lot in that time!) and looked into varicocele, spermatocele, hydrocele. From the ultrasound done back in September, I have a small cyst on my left epididymus, but that wouldn't be causing this on my right one.

Any ideas? My doctor has told me to take Naproxen, 500mg, 2x a day. I don't have Naproxen, but I'm taking Ibuprofen. He also told me to take hot baths. Those help, but the pain always comes back. Do you think another ultrasound might show something? I'm tired of throwing money down the toilet for unneeded tests, and I have minimal insurance right now, as I'm an entrepreneur.

One other kink in the story is that I took a bad fall onto my tailbone back in September, and re-injured it. I injured it years ago in a skiing accident, but my back seemed to get so much stronger last year, and all back pain went away, but came back last year once I started my desk job. My psoas muscle now feels very tight, and my anus gets dry and itchy and bleeds a bit when I wipe. This never happened before this past April, after the testicle pains started. Do you think this could all be related to the psoas muscle? I stretch out daily, including doing recommended psoas stretches, which help the pain, but it's still there. It's the "ridge" on the front of my right testicle that hurts/worries me.

Suggestions? Thanks guys, and I hope that we all find some relief!

Just to be clear, are you experiencing the front testicle pain that I am? It really seems to be referred, as it's worse after I've been sitting a lot (I used to work long hours at a desk job, and now I have a desk job and I travel a good bit), but it's better after activity. Is this the same for you?

I too experience the same symptoms you do.  I also fell on my tailbone about two years ago and have been experiencing the pain since.  I also find it hard to pass gas, and find that fiber seems to help keep movements regular.  I'm also a 25 year old white male.  I'm very interested in seeing an answer to this question, as all the physicians I have seen have said the same thing I've read here.  I too am on 800mg doses of Ibuprofen.
