Varicocele, Multiple Sclerosis or ?????

Warning: LONG post.

Here's my symptom list, aside from ones too infrequent or once upon a time to mention, ones that stopped (like my penis pain, that probably was my UTI) or ones I just forgot at this point.

Scrotum/testicle pain (admittedly alot less common/constant these days, like my brain gave up on sending signals for something that isn't getting fixed).

Erectile dysfunction.

Testicular atrophy (but they unshrink and re-shrink back and forth at random).

Completely numb genitals, pleasure-wise anyways, I can still like tell they're on my body though mind you.

Back pain on the left side up high around my shoulders when sitting upright that temporarily leaves if I lay flat on my back (I guess a little more like pins and needles than pain though? I don't know what to call it), it similar to like when your foot goes to sleep but more annoying and constant.

My brain/head actually tingles sometimes, might just be my hair growing or something I dunno, I just started actually taking care of it recently lol.

Fatigue.

Frequent urination (then other times nothing almost the whole day, depends on what and how much I drink, water goes right through me).

Have to switch feet or stand on just one foot for a sec when standing to stop the pain.

Leg/knee pain (usually just one or the other).

Butt pain when standing up.

Leg pain/stiffness after standing in one spot a while (like washing dishes).

Back pain when standing but lower (about in the middle).

Very little semen and it's watery (if I can force anything out) more often than not.

TERRIBLE Memory problems.

General brain fog.

Man-boobs.

Seems like I can't breathe deeply.

Can't add any force to Pee stream.

Peeing pink??? (blood??) at random.

I blow my nose almost every day without fail (only once or so in that day if I'm lucky), the snot can be anything from clear to yellowish, to blue-greenish, to brown-red/pinkish.

Hyperhidrosis (armpits sweat way too much).

The erections I do get only happen when I touch myself.

Scrotum/testicle "pain"(???), hard to tell which but probably both (this has gotten very uncommon).

Urge to pee that's way too strong for the small amount that comes out sometimes.

Strange Behavior(s).

Weird hand twitch when typing on computer sometimes, it's like a weird mix between my hand jerking/jolting but fighting itself not to at the same time, that's the best way I can describe it, can happen to either/both arms/hands at the same time.

Weird superpower where I can practically go on forever without drinking any water or just a few sips whenever I do actually get thirsty (possibly due to barely any every coming back out more often than having a fountain these days), seriously I thought that killed people.

Constipation.

Lexapro 10MG helps the pain and oddly makes testicles grow kinda but not for long/it still varies? (probably coincidence).

Similar lump(???) on both testicles (could just be epididymis, would explain the bigger left since they said I have epididymitis on that side).

Perineum pain sometimes.

Testicles always shrink before I pee/poop, even worse than they already were.

Almost none of these are constant and consistency varies from symptom to symptom, and I'm always better or worse at certain times and testosterone levels fluctuate so hmm?

Did testicle ultrasound, neurologist (unknown brain scan), endocrinologist (hormone test), ureteroscopy (dumb female doctor thought "my hole" was a little too small, that's what I get only mentioning peeing alot), x-ray (I said full body, don't know how much they covered while I was standing there), you name it honestly, never got around to MRI/CT scan type machines though because of what you have to do for insurance to even cover it, never got around to checking out my prostate either.

Only the testicle ultrasound showed anything, a harmless small hydrocele on one side (maybe both I forgot) that I would've never even noticed in any way (not even aesthetic trouble) and left side epididymitis but to me the symptoms (among other things, like no way for me to get the infection kind) don't even remotely line up, having a UTI just once and symptoms like peeing alot that could be any number of 5+ different things aside, and I'm not sure how different symptoms are for each kind (infection or just inflammation due to whatever).

I've eliminated other things that cause low testosterone/testicular atrophy like diabetes and sickle cell, I'd definitely notice if I had those and probably be dead due to no treatment for 3 years, probably would have been gotten the problem diagnosed in a hospital.

Do you think my iron deficiency anemia could have something to with it (doubt it after all this time of not even having so much as a energy shortage with it).

Interestingly enough I layed (oops, laid) on my back during the ultrasound and they say that can cause a misdiagnosis as epididymitis (SOMEHOW).

I'm considering multiple sclerosis due to some similar symptoms and cause I did have pneumonia and strep throat (I think) at the same time once and I'd say this all started shortly after that and my symptoms are changing/vary in severity kinda, better in some ways worse in others, but multiple sclerosis probably (definitely) isn't the only thing that can change with time, I was 16 back then and 19 now after all, and as I said earlier testosterone does fluctuate.

I'm basically done with doctors until I can at-least get a answer or two on here on what I should even look for.

Well, MS and a varicocele are very different.

Depending on what kind of brain scan you had, they would have seen the damage to the central nervous system to diagnose it.

According to the National MS Society, you have to have these to diagnose MS:

*Find evidence of damage in at least two separate areas of the central nervous system (CNS), which includes the brain, spinal cord and optic nerves AND
*Find evidence that the damage occurred at different points in time AND
*Rule out all other possible diagnoses

It's for sure a process.

Varicoceles don't cause symptoms anywhere except the groin area.

One thing that's concerning is that you think going without water is some kind of power or good thing. There is debate on how long you can go without water - some say up to 3 days, some up to 2 weeks but all agree that you'd be dehydrated. Some of what you are experiencing may be dehydration.

Excessive sweating is a sign of dehydration, and muscle aches and pains are another.

Iron anemia can absolutely cause some of these symptoms. Fatigue can be caused by both anemia and dehydration.

Maybe for a week, try drinking enough water, and see how you feel. You don't mention the anemia until the end of your post, so I don't know if it's bad enough for infusions, or just that you need to eat more iron, or take supplements, but if it's just to increase your iron, try that, too.

I don't think it will help everything, but it may help some things, enough for you to determine what's what, and what you need to investigate further.

Me: Not really, that was just a dumb joke based on how I seem to be doing decent enough not drinking water cause the bathroom trips are frequent enough to run anyone insane, just reminded myself with ONE cup lol, I'd be scare to go to sleep afterwards too it's that bad.

Drinking water in this state is honestly just not all that practical, I'm not sure how much my body is actually even keeping/using since I pee so much, I thought the pee was supposed to be just excess not 90% of what goes into me, especially when I shouldn't have any left over already.

Me: Ok thanks, probably not bad enough for infusions cause that sounds like I'd be very sick but I could be wrong, I thought it was called a transfusion? also I kinda felt better after eating like 2 burger patties a few days ago but it still wasn't a huge difference and didn't even last into the next day so I doubt food can cover it or at the very least I'd end up obese trying to eat 10+ pounds of meat (etc) per day to keep my body running right, I've gotta start supplements at this point.

There are different kinds of anemia. Some are where you just aren't eating enough iron, some where your body isn't absorbing iron. One of my closest friends has the kind where you don't absorb the iron, and she has to get iron infusions. (Infusion is when a different substance is injected into the bloodstream, like iron, and transfusion is when the same substance  - blood - is injected into the bloodstream. So you have blood transfusions, and everything else is an infusion.)

She doesn't get very sick. She gets very tired, irritable, brain fog, muscle aches, headaches. She gets infusions every 6-8 months, and her body tells her when it's time.

If you felt better after the red meat, that's great, but it won't be a long lasting change. One meal is awesome, but it needs to be a long-term thing.

You could have something going on, but maybe part of it is that you just need to take better care of yourself. Eat better, take some supplements - check the directions on each, and don't take more than directed, because too much iron is a thing, drink enough water, exercise, sleep, etc.

You're young. Make it a habit now and it will be sooo much easier later, trust me.
Ok I feel better after drinking some water a few days in a row to be honest, now it's still just the testicular atrophy, ED and low semen mostly but that's honestly still improved a bit too, my refractory period has even gone from 1-several days to a few minutes/maybe a hour or hour and a half.

But I'm still stumped about the epididymitis that I apparently have on the left side (that I can actually feel at times), I can't really find anything that acknowledges low iron as a cause of low semen online though, just low sperm, low libido and ED, my memory is still a little off but way better to be honest.

Still kinda have scrotum pain too though, I should've known because at the beginning it was really only these symptoms anyway.

Epididymitis is pain all the time. Are you feeling pain all the time, or just some times? I'm a little confused by that.

Are you taking the Lexapro still? Antidepressants can cause lower semen volume and low libido, as well as ED.

Keep drinking the water. It may help you metabolize the Lexapro. It may all just take time, or you may still have the sexual side effects from the Lexapro, if that's what is causing it.  

I take back what I said about improved sexual function, it's really the same old "mostly dysfunctional but lucky at random" that's it's been for 3 years now,  but I am better in some ways like not feeling like I'm gonna fall over/being VERY stiff after standing a while and things like that.

After being well hydrated for a few days here's a update on my symptoms, luckily my troubles really are mostly sexual and the rest was just dehydration, that's a good sign, pretty likely it's just iron or something else fairly minor right? to think it was just low iron and I could've fixed this basically the day it started instead of ALMOST HALF A DECADE later is a bitter-sweet feeling.

Eyes blur (random but seemingly getting more common and usually my right eye I think, including when I first wake up but I guess that's normal enough).

Same weird upper left shoulder/Back pain/pins and needles sensation (tight/sore muscles or something?).

Fatigue.

Testicular atrophy but they grow back at random.

Penis shrinks and grows too (not talking about erections I don't really get).

Erectile dysfunction.

Very Low and watery semen.

Numb genitals.

Orgasm all but impossible 97% of the time and I don't think it's just because of low semen (ejaculation and orgasm aren't exactly the same).

Refractory period is 1-several days unless I get lucky (I never truly "feel like it" I just force it).

Genital "rigor mortis", seriously everything feels glued in place most times and just generally off including the skin texture, my scrotum gets it the worst including always being high like I'm freezing cold and actually disgusts me.

Still blowing my nose like I have a cold (color usually clear, but once was brown like the chocolate I was eating for some reason, used to be blue-greenish or even orange-yellowish or red-pinkish).

Peeing blood (very random).

Man boobs.

Random fairly intense stomach pain.

Was nervous???/twitchy as all heaven today earlier (do people say that? I just did but I digress).
I haven't really had a Lexapro in a while now and I think this all started before I ever touched a SSRI, so although those can cause sexual side effects I'm leaning towards it being something else, I don't have testicle/scrotum pain all the time on either side but at random (usually the left these days).

It used to constant or close to constant on both sides for some reason but it's been calmed down for a long time now, I used to have penis pain too but that stopped some years back (I think that was my UTI too).

Considering the long half-life it could maybe be left over Lexapro side effects but I haven't really been getting progressively better so I doubt it, terrible one day and very slightly better the next day in that aspect isn't convincing me it's that, I've also been off the Lexapro since November 2019 I believe (back when I made my first much less detailed post and therefore didn't get far).

Every once in a while I take one but still, if anything it makes me feel better sexually along with the obvious (serving it's purpose), I still never get past that first ejaculation even then though, I also wait five days for it to be out of my system before the next one if I decide to take another, also for what it's worth I never went past 10MG either.
Low iron could also explain the dark circles under my eyes too, not exactly bags but just dark circles.
So I think a lot of it could be chronic dehydration - it might take awhile for everything to get better. Try and drink several glasses/bottles of water a day. Experts say to aim for 64 ounces, and of course, there's debate on if you need that much, but I think you should just aim for as much as you can.

I don't know which of your symptoms are iron related or dehydration, and only time will tell. Dark circles can be both, for example, or genetic. Or from allergies, for example.

Testicular atrophy that randomly grows back and genital rigor mortis - sexual side effects from SSRIs can last a long time, but I don't know if it would be from that since you weren't on it long, but since you still take it occasionally, maybe it's still doing something. Maybe it's something else. It could be iron. Time will tell on that, too.

I think once you get healthier overall, you can better tell what's still a problem that needs more investigation.

I have fairly consistent left flank pain too but it seemed to just start yesterday, do you think it's just like a muscle spasm or nutcracker syndrome? and the "epididymitis" is actually a misdiagnosed varicocele caused by that?

That would explain the peeing blood too but it seems to be when I move/lay a certain way so I'd say it's just (a) muscle(s) even though it's still here today, I swear the epididymitis was really just a fake diagnosis and it's just big on that side for some reason so it seemed like a bad thing.

After all a girl did the ultrasound and it wasn't a urologist just a random hospital I got referred to because they had the machinery I guess, I don't know who looked at the results though, male or female etc.

Also my anemia never bothered me before, would it really just suddenly make my balls shrink etc. as soon as I hit 16? I don't even feel like I hit a huge growth spurt anywhere then to make the shortage a bigger problem so I don't get it.

I really only have the fatigue symptom, peeing blood, peeing alot and maybe the random periodic eye blur is related, maybe cold hands and/or feet every once in a while but it's winter so that might not count (which maybe just means I'm just "built different" or something lol).

Some research seems to say low iron (as well as too much iron) can cause hypogonadism/testicular atrophy, low libido and low sperm but I'm sure if I should just take that with a grain of salt or not, just search iron deficiency/iron deficiency anemia testicular atrophy and you'll see what I mean.

Seems like a symptom but a mostly unknown one that you have to specifically search for just like with peeing alot and peeing blood etc. if not listed with the other general symptoms if you just search "iron deficiency/iron deficiency anemia symptoms.

Sorry if it seems like we're going in circles at this point, I just want to make sure this is something I should look for at all before I waste my time again, and what I should I should look for while I'm at it, I'd love to kill 2+ birds with one stone not make 5-10+ appointments when it could've been 1-2 or so.

You may be urinating more because you are drinking more water. On average, people urinate 6-7 times a day when they drink about 2 liters of fluids. (2 liters is about 68 ounces.)

If you drink that much, or around that, and urinate more than 7ish times a day, it's considered frequent urination.

Flank pain can indicate a kidney infection or a kidney stone - any kind of kidney issue.

Unless pain is super sudden and really severe, or accompanied by a fever, nausea or vomiting, you can usually give it a day or two to see what happens. If it's just your body adjusting to this new water intake, it will settle down. If it's an infection, you'll know it.

Testicular atrophy - did someone tell you that you have that? Hypogonadism occurs when sex glands called gonads produce little, if any, sex hormones.

Testicular atrophy is the reduction of size of your actual testicles, not your scrotum.  They can seem to shrink in colder temps and relax in warmer, so they can appear to be different on different days, or even at different times on the same day.

Have you been diagnosed with low iron by a blood test?

I think you should get checked for a kidney or urinary tract infection, which your regular doctor can do, as well as get checked for anemia, which your regular doctor can also do. When they test your urine, they should be able to tell if you're dehydrated as well.

Have you ever talked to a doctor about your testicles and genital numbness/low libido/erectile dysfunction? If not, do that, too. Your regular doctor can start with an exam, and check your testicles for any problems, confirm epididymis (were you treated for that?) or refer you out to someone.

So for the doctor:

* Urine test for UTI/kidney infection/dehydration
* iron test
* testicle check

Seems like a great place to start. :)

Honestly the peeing seems to be getting more reasonable, I remember it used to practically ruin my whole day it was so bad, but I'm not so sure it's gonna stay that way, and I can't tell you how much I've been drinking really because I don't measure it, but I'd say it's a good bit, I might try to get back to you on that.

Yeah that's honestly piped down already and really only happened when I laid on my back, no fever nausea or vomiting.

No but I feel like I really don't need to be told, they were probably at-least a good 2 or 3 times bigger when I was 13, now I'm 19 (not 91 lol) so that's very backwards and makes no sense and it seems very obvious to me, nobody ever said it because they probably thought it was normal and didn't want to hurt my feelings if I was just small by default but it's definitely not normal for me although it's a bit hard to remember what they used to be like at this point.

No, I've done blood work MANY times over the years and nobody ever mentioned iron, so either it's fine nowadays or that never really was a big problem for me, at-least not anymore, back when I was 12 I used to eat ice cubes like a real late night snack though lol, but otherwise I would've never guessed, had no issues at all, not even fatigue.

Honestly no, I was always too busy sugar-coating it (as in just not saying things) due to at-least one parent being in the room, they know about all these things yeah but I could hear the teasing at best or being told "you ain't got no business blah blah blah" at worst from a mile away but I could've been wrong about that the whole time honestly, And no the epididymitis was never really treated because it's "ghost epididymitis"/"ignored epididymitis" I got the diagnosis but they never gave me antibiotics for it or anything.

That was just that for some reason like they didn't even tell the doctor afterwards so he could prescribe something, probably need to try another doctor/place altogether in all honesty, I don't reckon I ever did see the hype behind them/bias towards them in my family, guy always seems to be running anyway and talks about 100 MPH.

Alright got ya, but when you say testicle check do you mean ultrasound or just poking around to see if anything feels odd?
You know what? I actually just remembered I got a shot for the epididymitis and got a typical 14 day doxycycline hyclate course the same day or whenever the prescription was ready and the doctor said come back if I still have pain and what-not but I never did go back because... I don't know, I think because at that point I was still being a smartass and thought just because I wasn't painting my room white yet/barely had any difference instantly it was a varicocele instead.

To be honest though I kinda felt something when I got the shot (random unprovoked erection that stuck around a good minute or so) and I thought my testicles felt better but thought it was all just my imagination because I wanted it all to be over with (placebo or something other more fitting name), I also feel like the antibiotics didn't do anything to be honest, but I think they're known for easily penetrating, especially in certain places, such as the prostate, not sure about the epididymis/epididymitis.

I also used bee propolis drops under the tongue, 30% and 70% from Amazon and it was another case of it seemingly doing something but not for long, to be fair I might've been licking it all up with my tongue and covering my teeth with it because I couldn't really feel if I was really putting it in the right place and wasn't sure if I was squeezing hard enough to get the other drops to come out since I couldn't feel them make contact with the "palate" or whatever they call it (I definitely always covered my teeth with the stuff and sadly other things lol).

Before it even had a real chance to absorb, so that might be my fault, that stuff should definitely work when used that way, it goes right to your bloodstream and to my knowledge no bacteria will/can even resist that, it's nature at it's finest and it's not homeopathic medicine type stuff, it's scientifically proven to work.

The fact that both sides are shrunken and seemingly non-functional/very close to non functional is what made me think it was varicocele instead, can varicocele even be seen as epididymitis by accident?

I also don't understand where the hell I could've gotten epididymitis from, I did have a UTI once though I guess.
You know what? I know damn well it's not epididymitis, it's just not even remotely possible for a dude that never touched a woman and had one UTI that probably got knocked out long before it could spread anywhere, mom conveniently had leftover doxycycline from/for grandad that I took before even going to the doctor and everything.

Varicocele and orchitis causes atrophy but I didn't get diagnosed with orchitis on either side, meanwhile here I am with "epididymitis" on one side and my whole "life" is ruined.

Notice how it's also specifically on the left side but shrunk both testicles and it started when I was 16? they're usually on the left side, if not it's usually a indication of a bigger problem, and they start between the ages of 15 and 25, wow what a "coincidence"

I was so damn close but nooooo I had to not even get diagnosed to see if the doctors would even take me seriously past that point and feel the need to get it fixed, or do embolization instead of surgery without me having tot threaten them about it or something, which from what I've heard.

They probably wouldn't take me seriously, especially with my "luck" they couldn't even diagnose it right, whether that be due to me lying down during the ultrasound or not.

I remember going to a urologist afterwards and apparently my PCP faxed the results to him already and he was like "no varicocele on ultrasound, can't feel it, you're cool" and probably wouldn't have even did another ultrasound even id I worded it better (I said "wanna check again?" that could be taken as asking him to poke at my scrotum again).

Speaking of my scrotum it probably "conveniently" did that shrivel up really bad and/or turn into leather/elephant tusk thing at the perfect timing so he genuinely couldn't feel anything, hell sometimes I don't even feel it.

Also I'm laying in this bed right now which a actual "squirming???" feeling in the left side of my scrotum, "worms" on a whole new level, and I had the random dull kinda squeezing pain high on that side when I got up to go to the bathroom not too long ago too, about where some of the "worms" are.

And as I said before, me obviously being bigger than 12 year old me aside, I don't crave ice or anything and apparently no sexual dysfunction is even listed as a symptom of iron deficiency/anemia except in random studies on sites like pubmed and for all I know I shouldn't take those seriously even remotely.

I know nothing about those such as how to read them or what kind of studies I should actually believe, what's actually causing what/a big coincidence they noticed and accidentally made something out of nothing etc, only ED is ever mentioned but it's really more of a fatigue side effect, not even listed as a common symptom worth mentioning.

So yeah, scratch epididymitis and probably scratch low iron (for the sexual dysfunction anyways) unless they just somehow never noticed it does that after ALL THESE YEARS, not gonna mention diabetes or sickle cell because those are obviously eliminated for obvious reasons, we haven't even mentioned them since I think my original post.
I meant to mention sickle cell, only to point out that it's only found most commonly in Black people in the US. I don't know your race, but if you are in the US, and aren't Black - and you may be - it probably isn't sickle cell.

As I said before, your scrotum wouldn't look smaller with testicular atrophy, but could it be that at your younger age, things looked different because your penis was smaller and as you grew, the proportions changed? Now your penis is bigger, making your scrotum and testicles appear smaller? They may also feel smaller in your hand that has grown bigger.

Just have your doctor check your testicles. It's a good idea at your age, because you are starting the age range (17-35) for testicular cancer, and you need to know what is a normal feel for you, so if you ever feel something different, you'll know when to get it checked out.

I understand how it is to talk to your doctor when your parents are sitting right there in the room. Maybe you can tell your parents that you want to talk to your doctor about your testicles because you read that you're at the age for testicular cancer, and you want him to show you how to check them or something, and you don't need your mom sitting in the room for that.

Ask them about the worm feeling. Maybe what you are feeling is normal.
Ok, as far as the sickle goes I'm actually in the US and black so it's honestly a decent possibility especially considering the blood pee I guess but I feel like I would've had more severe symptoms of some kind if so, especially these past 3 years since I have been having sudden troubles, the only pain I have though (at-least right now and aside from scrotum/the most frequent of them all) is the weird thing going on in my upper left back/upper-back side of my left shoulder, not that I'm complaining that I don't/didn't have bigger/more noticeable troubles lol.

But one thing's for sure I definitely don't have decent parents that would point out anything pre-existing that they know about, they'd much rather play games and I like I'm a keraazzzyy (crazy) whacky dude for saying "hey I don't feel 100%", all they ever said was too little sleep, too little water or sleeping at the wrong time (obviously no such thing) or "iT's jUSt GROWiNg pAIns/pUBerty/gRowing up" they always had to turn it into a "gotcha" every single time (but that's more so dad, mom was the one that basically just went "silly kid, Twix are for kids" so to speak AKA actually using the word crazy).

I honestly feel like despite feeling weird (unnatural even) 95% of the time my scrotum is still the same size, but the testicles on the other hand... oh boy... the scrotum to testicles ratio is HORRIBLE, I'm talking something along the lines of 10:0.9-1 (let me specify the scrotum is the 10+ and testicles are the 0.9 or 0.something-1 at best) as I said before it varies, sometimes they're basically not there (and I always have to touch them to remember they're still there regardless cause they're small and don't tingle or anything, not even pain, that's all in the scrotum).

Especially the left testicle, I can be standing at the toilet and it shrinks worse than usual or at the very least gets covered up by the worm things (hard to tell) because they got bigger temporarily maybe and the left side of my scrotum actually deforms itself all at once, but everything always goes back to "normal" (in comparison) seconds later like nothing even happened, and it seems to be getting more frequent lately oddly enough.
What I mean by the ratio thing is my scrotum really is practically swallowing them, and lack of a better way to put it my scrotum is like overly loose because my testicles are a little further back than they should be I think.

But that's probably just extra room because they shrunk and got soft or not a thing at all like I think it is, and they aren't exactly firm either these days, the left one is consistently smaller (probably was the smaller one to begin with, one usually is), I'm 19 now by the way.
Listen, I'm older than you - a lot - and have seen my share of scrotums. They WIDELY vary. I haven't paid much attention to the ratio of things, but a doctor has seen yours, correct? They'd know if the ratio was bad in some way.

You are 19 now. It doesn't matter what your parents think. Are you out on your own yet? In college or working? If you're in college, does your school have a student health center? You could go and just get a basic exam to see if everything is "normal", and mention the worm feeling/things.

I'm sorry your parents aren't safe for you.

In any case, I think seeing a doctor is your next step. We talked about this earlier -

* Urine test for UTI/kidney infection/dehydration
* iron test
* testicle check

I think you just need to mostly see if things are "normal", and make sure you don't have a UTI or kidney infection, or anemia.

I didn't say anything because I was kinda mad but I just realized something, let's be reasonable here, if I had a kidney infection for 3 years I think I'd notice, I'd have barfed at-least once or just straight up gone septic or something that's not adding up at all either, at that point even my parents would be like "ok this is serious, let's do something", I'll do the all tests regardless but still, and to be honest I don't think the doctor ever saw my "stuff" before all of it all but melted into a disgusting puddle of mush.

Especially not the 2nd one I go to in the same building (the lady one) because I usually go to the guy one 99% of the time and I never really had a reason to have any of that checked before things went south and they never did it just for the heck of it either, And I swear if they were even remotely decent doctors/a decent staff in general somebody would've noticed any kind of iron/blood problems after all this time and at-least one-two blood work tests per year, I also have zero UTI/kidney infection symptoms really, I'd know UTI symptoms from experience and the kidney infection probably wouldn't just happen with no warning/prior UTI or some other infection.

It seems like you're just trying to make me cover the same old ground several more times again and that's exactly what I don't wanna do (except to verify if my doctors are truly quacks or not, that'd be cool I guess), I wanted some answers on what the issue probably is not a bunch of tests I should go do for the millionth time that never got me any meaningful results before, the epididymitis diagnosed still sounds like bull to me too, because of the simple fact FROM WHERE WOULD I GET THAT????? and nothing I tried caused ANY IMPROVEMENT?????

Not saying you don't know anything but I know how my junk used to feel and this isn't even remotely, even though I can barely even remember the good old days at all after 3 years, it's not exactly rocket science or brain surgery to me, I literally want to jump out of my own skin at this point and bleeding out/the pain is quite possibly the only thing keeping me from chopping it all off, especially since I'm losing hope now considering it's been 3 going on 4 years this year (I repeat, ALMOST HALF A DECADE) and because even if it's just a simple varicocele it's hard to detect and nobody will take it seriously either way and I'll probably just have to live with it, in fact you seem to be ignoring the possibility right now yourself saying it doesn't sound likely because... reasons.

Let's forget about aesthetics and how weird and "so bad I wanna just cut it all off" everything feels for a minute and let's acknowledge the fact that I was literally fine for years but one night I started my "usual routine" and was still mostly functional as far as I can remember, but when I "finished" it was basically just a little drop then it just snowballed worse and worse from there until I was shriveled up, couldn't feel anything, couldn't ejaculate period or get it up, the sheer suddenness of it may or may not serve as some kind of a hint.

I'm not trying to be mean or pester you too much or try to coax answers out of you that you don't have but seriously, 15-30 doctor visits over the course of 3 years is already way too much as it is, I feel like mom might be right if I'm not careful at this point I'm gonna mess around and get sent to the "funny farm" especially if I don't get off of the urine tests (probably not but I digress, I'm about to send myself to the funny farm at this point), that's the test I've already done 10+ times, everything else was a one time thing (so far) at-least.

Too many times I've told myself "today's the day, here we go" and then nothing, I even went to see if I had fungal epididymitis one time and the doctor was like "nah, that's all but a myth for men" as if he couldn't at-least check, so god knows I won't have any luck going to him for a varicocele, I've read the horror stories about doctors telling patients to their face it's not a problem or doesn't cause low testosterone or asking if they want kids implying they don't give a F and won't help you if not and I don't even need to explain why that's beyond disrespectful.

Not their business or important for starters and with my "luck" in my 19 years of living I definitely won't be one of the one's to be taken seriously and get somewhere, not without going to 100+ different doctors first, locally, out of town and overseas.
You have a point though, I woke up today and my neck was really weirdly stiff for lack of a better way to describe and when I moved I felt the pain from the left side of it all the way down the far left side of my chest (at-least my upper chest) and now slightly above the middle of the right side of my back hurts when I move my right shoulder a certain way, sometimes when standing I can't breathe deeply, and when I was standing last night at the toilet my right knee hurt for some reason, all types of things that could be simple muscle pains/spasms (maybe not in the case of the knee though) or sleeping a little funny but I don't know that.

It's all so out of nowhere and "once a month" "once upon a time" "there was this one time" etc. I can't even decide if they're worth pointing out and that's not making a actual genuine diagnosis any easier to say the least, also the symptom list keeps getting bigger based on these random occurrences and I'm not sure any doctor has the patient (oops patience, no pun here lol) to read it all and actually pay attention instead of just skimming through it (would maybe take 5 minutes at best though).

Whatever this is it's quite ridiculous and I guess you're doing the best you can with something with 10+ symptoms, most of which turn on/off like a switch or vary in severity, and you're probably right about what tests I should go for and it not being a varicocele, but I'm desperately trying to make sense of the worms and epididymitis diagnosis, probably just a big epididymis that's maybe accentuated by the shrunken testicle but the quacks didn't know any better and neither do I and I've done all these tests so many times before I feel like it's pointless.

But they really could just be useless doctors missing everything, (scarce and not so detailed/completely wordless) Google reviews haven't helped me much in that aspect although they do have 3.9 stars overall and one said "This is the worst damn place u can ever go. I will rather Google my problem than to go in here. The practitioners are dumb as hell and do not know how to treat anybody" and another one said they only go there for one specific doctor (the one I use that still doesn't seem like anything all that special to me).

I think it may be autoimmune since it all started after/about the same times I had pneumonia back in late 2018/early 2019, maybe multiple sclerosis.
You call me rude, but somebody is unironically trying to tell me over the internet that I don't know what my own testicles/scrotum should look/feel like, the same type of BS my parents pull like I haven't had plenty of years to figure these things out and just because I don't show them what's left down there for obvious reason and they'd play dumb even if I did, I was never this small, in constant pain or completely non-functional before, why is it that I'm supposed to think or pretend this is normal when it's not? I asked for help not the "you're crazy, nothing's wrong" schtick all over again, I leave my parents alone cause I can handle my own business now anyway and I almost don't care anymore and now someone else decides to take on their former role in the "adventure" (through hell).

I really am just at my limit at this point like anyone would be, I don't think anybody can blame me for cracking up a little after 3 years of this+ my parents thinking I'm stupid, I'm a step away from saying "screw it, time to live with it" (doubt that would stand for long because of the pain if nothing else, I'd start crawling up walls and ceilings sooner than I'd truly get over it), I'm a 19 year old grown man but it's more like I'm 119 and I wish I could say my problems are always just sexual but sometimes that's not even the case.

Even if I was small by default I'd still have some kind of function, my scrotum would still be normal and I'd know what normal is/was for me, nothing would get any smaller (instead of everything, including my scrotum the more I think about it), and I wouldn't always be hurting and that's the bottom line, nonsense like this doesn't change the truth, I know what I should be like, I went from "3 per night every single night, 365 days a year" to "one per year if I'm lucky/don't force myself out of sheer denial even after 3 years".

And don't even get me started on you, you never helped but now you just pop up out of nowhere to call me "rude"? come on now, how about some suggestions aside from the same old same? that's all I ask, nobody seems to want to name what the symptoms could be or confirm/de-confirm what I say, alot of questions I asked where basically just ignored like when I pointed out the squirming in my scrotum, except for in the first reply but she didn't even say what sounded like symptoms of anemia/dehydration individually, no specifics/detail or elaboration at all.

I can't reply directly on the site but rude in what way? your definition is probably different from mine anyway, I feel like I'm being fairly calm after 3 years of "you're fine" and now "do the same old tests again and if something magically changed even though your symptoms don't even slightly resemble these things and you've been there a million times", just seems kinda non-sensical to me.
As usual I'm honestly mostly better off Googling a bunch of different and trying to piece something together and unfortunately no matter what it seems like I'm gonna have to face the music and do the same old tests again (and some I haven't) and just pray??? somebody finds something and doesn't have the gall to say "screw you that's not a real problem, suck it up you baby" because I'm not trying to have children when I don't even have a job let alone my own place and I'm just now learning how to take care of my hair all by myself, just learned how to tie my shoes at 18.

Just learned clothes tailors even exist at the same age, just starting to teach myself how to wash clothes with no shrinkage, mostly teaching myself how to cook at-least as far as recipes etc., notice a pattern here? I always have to take the initiative because I allegedly didn't want to learn "blah blah blah" years ago so now they figure they shouldn't even try to teach me ANYTHING if I don't bring it up at-least (they don't know squat anyway, it's a cycle that I've gotta break), mind you they've always pestered me about combing my hair (only when I go out for the sake of their "royal image" they don't have) and about wearing clothes but never even tried to teach me how to make it comb-able to begin with or buy the right size clothes or get them tailored.

But the doctors won't care if I don't wanna get someone pregnant as soon as I can so I can pay child support for a child/children I'll never even get to see with some girl that I just found yesterday at Walmart (first woman I'd even know outside of my family) "lol".

Regarding your issue, I don't know much about the topic, but wonder if you have run across descriptions of Morgellons disease in your search. In the Women's Health community we hear about it when someone writes in about getting what seem like threads coming out of the pores of their skin. But it also comes up online when the topic of a squirming feeling in the scrotum is searched. Unfortunately, even if it seemed like Morgollons is what you have (and the list of symptoms is not a total match), the disease is not well understood. The Mayo Clinic site has an accessible and informative article about it, but notes that there are more questions than answers about the condition, even to the basics of whether the condition is psychosomatic or not.

Realizing that's not much help, all I can offer you is good luck in your search.
