severe lower left abdominal pain

I have a younger brother who is 17 years old, who for as long as I can remember has always had stomach problems. Our mother had stomach problems where she'd eat something, anything, and it'd go right thru her either by vomiting or going "number 2", but not a healthy, normal bowel movement. And both my brother and I have had the same problems. Mine aren't as severe as his. But maybe for the past 2 years or more, he'd wake up feeling nauseated, and throwing up, sometimes with blood, or having diarrhea, that usually resembles stomach acid, or hes constipated, and it comes out black, which I heard is digested blood. Also, (sorry if I get too detailed) he said sometimes when he's done and wipes, there is actual red blood on the toilet paper. He says that he wakes up with pain on his lower left abdomen, and its right by his hip and it feels as if it kind of wraps around towards his back. He says its swollen, and he can see it and I personally have noticed that he's lost a lot of weight, but he's always been thin. Hes very stubborn and doesn't want to go to the ER or doctors or urgent care because hes afraid and assumes theyre going to want to cut him open. I keep trying to convince him to go but hes just so stubborn. I've tried to google symptoms and see if I can find anything that resembles what hes experiencing, and when I start to read anything he just freaks out and says he feels like hes going to faint. I am concerned because he is so young, but he is such a stubborn brat and I KNOW the only way to find out whats REALLY going on is to seek professional medical attention, but I am just asking for ANY advice, opinions, even experiences from anyone. Please. Thank you.

Hi,
   It could be Celiac disease, chronic amoebiasis, Intestinal parasites, intestinal ulcers ....., only a proper diagnosis by a Gstroenterologist can find out the real problem. After diagnosis, if surgery is advised, any alternative to surgery if possible could be considered.

Needs to go doctors could be numerous things UTI, problems with gut or intestines but could be appendix depending how long pain been there
