Do you experience any other symptoms aside from the headache? How would you describe the headaches? Are they constriciting, stabbing/sharp, dull, or throbbing? Where in the head do the headaches occur?
It would appear that the findings on MRI can possibly be related to the headaches you have been experiencing. However, other causes of the headache cannot be ruled out at the moment prior to having a full neurological assessment done.
Your neurologist would probably prescribe you some muscle relaxants (if muscle spasms are indeed considered the cause of your symptoms) and pain medications and would direct you to a more appropriate physical therapy regimen if the current regimen is not helping ease your pain.

I have this exact problem. I went to physical therapy every other day for a year. I still have the problem but can no longer afford the therapy. I work on my posture to correct the problem but there isn't much, short of surgery (dangerous at this point) that they can do. My thoughts are with you, this is a painful condition.
Do you happen to have a concave inbetween your ribs as well? Have you been diagnosed with TOS at all?
