Chronic and Acute Extremities Pain

I have seen two Neurologists, a Rheumatologist and several GP's, and none have any idea what is wrong with me.  

SYMPTOMS:
1) Chronic hand, arm, foot leg pain. This pain started roughly 3-4 years ago, initially as a pain in the center of each palm and and then in the center of each foot, gradually spreading to all parts of the each hand/foot as well as each arm/leg. The pain is not limited to joints; it feels like it is in the muscles as well as the bones; very achey/crampy in nature.  I am not sensitive to any actions, positions, or touch.  The pain is in a constant (non-specific) rotation; for a few seconds my feet will hurt; then a part of one of my legs; then a part of my arm, etc.  There may be pain in more than one area at the same time.
2) Acute leg pain.  This pain happens very sporadically and happens in the same exact spot each time (the middle of my left leg in the hamstring area).  When the pain hits, I feel like I have been electrocuted or stabbed and only usually lasts for 90 seconds or less.  My first episode happened about 3 years ago and have had approximately 50 episodes since.  I am usually not active when it happens.  After the initial jolt of pain (9-10 on the pain scale), I will usually have several less painful jolts that happen within seconds of the initial pain.
3) Pins and needles in my arms/legs and hands/feet.
4) Fasciculations in my arms and legs.

TESTS/FINDINGS:
1) MRI's on lower spine, left leg, and brain: normal  (brain MRI was about 3 years ago)
2) Nerve conductivity test: normal (3 years ago)
3) Rheumatology blood: normal (6 months ago)
4) GP/Neurology blood: normal (3 years 1st Neurologist; 18 months ago 2nd Neurologist; 3 years and 6 months blood)

I am early 40's, male, good physical shape, no health issues (other than described here), no meds, etc.  The pain has gradually gotten worse over the years and seems to worsen during times of high stress.  Has anyone ever heard of these symptoms, especially upper AND lower extremities?  Any ideas would be appreciated. My doctors seem to all be at dead-ends and I do not know who to turn to next.

How are you?
There is a possibility that these symptoms are due to benign fasciculation syndrome. It is characterized by muscle fasciculation, pain in the muscles and paresthesisa like pins and needles sensation and muscle cramping.

These symptoms occur commonly when the person is unduly anxious or under stress.

Can't help but to say i have the exact same thing. 100%. same series of docs same results. in other words, nothing is wrong with me but its getting progressively worse.
