Your muscle & toe cramps could be associated with localized injury, arthritis( reactive or rheumatoid etc.), and gout etc. Calcium deficiency could be possible as your PTH level is high but you need to get it confirmed through blood test prior to treating the toe cramps through Vitamin D supplements. You may require an orthopedic surgeon to reach to the root cause of toe cramps.
