off balance dizzy feeling

Well, this may be long so please bare with me. I am extremely frustrated and I do not know what else to do.I am 30 years old female with no health issues or a history of issues.

Around June 2016 I started to cut back on smoking, I was a heavy smoke for a few years. I quit for a few weeks, but went back to smoking at least 3-4 a day. July I started to get this dizzy feeling while I was smoke free and assumed it would go away, which it never did.

Here is what I have been tested for:

ENT - all normal, been three times.
Neurologist - CT scan and MRI - both normal, they assumed it may be migraines that are silent with associated vertigo.
Cardiologist - echo, holter monitor showed within 24 hours that I had 50 extra heartbeats but he was not concerned, and blood work for inflammation and etc, all normal.
Primary tested me for: allergies, blood sugar, blood work, blood pressure, thyroid checked, iron, etc. All was normal!
Eye care center: I have astigmatism and vision is 20/30 and 20/25. Not sure if they tested me for anything else, but whatever they did do it must've been normal.

My primary doctor thinks its anxiety, but its an off balance feeling to. I feel it most when I am standing washing dishes, washing my face - in front of the mirror I feel it a lot, in the shower its bad. I never fall over but the feeling comes in waves making showering nerve wracking.

All these tests are coming back to normal, but I dont feel normal. I am now 55 days smoke free, finally quit for good and I cannot take it anymore.

When I am sitting, it will come with waves, almost as if someone is pushing my head back but there is no head motion. Standing it feels like I am on a boat.

Just recently I started to get nausea at night, this is a new symptom. I do notice that my right ear feels/sounds like there is extra ear in it, especially when tilting my head to my left shoulder. But as previously stated, my ENT said there was nothing wrong. Maybe I need a second opinion, I do not know.

Any advice or anything else I can be tested for, I would be happy to look into.
