Hand Weakness and Muscle Twitches

I'm very worried about ALS but I'm hoping that this has gone on too long already for it to be that.  About 1 year ago I strted noticing a problem with my left hand - specifically the middle finger did not extend fully and was significantly weaker than the others.  

Today, My entire left hand (with the exception of the pinkie) is affected.  When I place my hand flat on a surface, I am unable to lift any of the fingers properly - some of them not at all.  In addition I have frequent muscle twitches and/or spasms in the left forearm.  No other limb seems to be affected.  I had physical therapy for three weeks with no improvement in the ability to move any of the fingers.  

I have an appointment to see a neurologist in early November.  WWhat is the possibility that I have ALS - and second, what, if anything should I harass my Dr (PCP) to do prior to my appointment with the neurologist.  Thanks for any info.

Sound like a cervical disk. Not ALS. ALS is very progressive. You don't have it.

Oops - I forgot a few pertinant details.  I'm 50yo Female.  And my left hand has no pain or loss of sensation - no numbness or tingling.
