Pain following Cubital Tunnel surgery

I had cubital tunnel surgery 7 months ago (Brachial Plexus hyperextension injury).  1 month prior had a scalenectomy, and 1 month post had guyon's canal surgery.  The inside of my elbow about 3 inches in all directions surrounding the incision and also inside my arm to the bone are constantly in terrible pain that increases with activity to the point where I can not touch the area.  I have stuck to my PT/OT home program, and followed my doctors orders. My neck and hand did react positively to the surgeries, my elbow is not progressing.  Just the opposite, the more I use it the worse it gets.  The pain is searing, constant and the area around my scar seems perpetually swollen and hot to the touch.  My doctor seems frustrated with my lack of progress, and is not receptive to my reports of worsening pain and requests for help. I am hoping you can give me an idea of what my symptoms may be indicative of.
