Severe migraine w/nausea

My 15 year old daughter has had severe migraines for as long as we can remember.  They have slowly gotten worse and more frequent.  She is now having daily pain.  She is becoming severely impaired by these.  There seems to be no relief in the long term so far.  We have tried many medicines.  Some do nothing, while others work short term and then stop working.  

It's likely to be beside the point, but her father died of an anaplastic astrocytoma... glioblastoma... killer tumor... in Feb of 1998.  This is scary, but not likely to be her issue, even though we keep an eye out for this.

Blood sugar and physical activity seem to be an issue.  However, she awakens with a headache each morning now.  She can be relatively ok and then she suddenly starts hurting.  Her most recent MRI (two weeks ago) shows no abnormalities, except a 1mm cyst in right maxillary sinus.

Her meds at this time are Topamax XR and Lortab with Compazine.  Pain relief with Lortab is mild and for only about an hour at best.  Stomach upset is common and Compazine effectiveness is also waning.  Topamax effectiveness is also waning.  We've tried Zomig, periactin, Imitrex tabs and injections, and I can't remember what else.  The major issue is that she can't swallow pills, no matter how hard she tries.  (We're working on that issue with M&M's.)  

She's missing so much school that it's not even funny.  I'm missing work.  We don't know what to do next.  She has an appt with a neurologist on June 4th, but she hurts NOW.  

We see the PCP regularly, but she said she's at a standstill until she can swallow pills.
