Hi there. In downward cerebellar herniation or coning, the cerebellar tonsils move downwards through foramen magnum causing lower brain stem and upper cervical cord compression. This may even cause dysfunction of centers in the brain responsible for controlling respiratory and cardiac function. This tonsillar hernaition is also called chiari malformation. The currently acceptable by radiologists as chiari malformation is cerebellar tonsils lying at least 5 mm below foramen magnum level. There are many possible causes of tonsillar herniation like spinal cord tethering or occult filum termiale which could pull down the brain stem and surrounding structure. Malformed posterior fossa allowing less space for cerebellum, hydrocephalus. Connective tissue disorders like ehlar danlos syndrome can be associated. It may manifest as abnormal posturing, decreased consciousness, low Glasgow coma scale, dilated and inresponsive to light pupils etc.
Treatment involves removal of causative mass and decompressive craniectomy.
